THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Iraq Campaign Medal vice the Afghanistan Campaign Medal.

2.  The applicant states she served in Iraq and not in Afghanistan.

3.  The applicant provides copies of:

* DD Form 214
* Active duty orders, dated 11 July 2010
* Orders, dated 29 November 2010, for Operation New Dawn in Iraq
* Release from active duty orders, dated 5 November 2011

CONSIDERATION OF EVIDENCE:

1.  At the time of her application, the applicant was a lieutenant colonel in the U.S. Army Reserve.

2.  Orders 10-192-00013, 307th Medical Group, dated 11 July 2010, ordered the applicant to active duty in support of Iraqi Freedom.

3.  Orders LS-333-0234, Joint Base Lewis-McChord, dated 29 November 2010, deployed the applicant on or about 4 December 2010 to Qayyarah, Iraq in support of Operation New Dawn.

4.  Orders 309-0003, Joint Base Lewis-McChord, dated 5 November 2011, released the applicant from active duty effective 14 December 2011.

5.  The applicant's DD Form 214 ending on 14 December 2011, shows:

	a.  award of the Afghanistan Campaign Medal with campaign star;

	b.  no award of the Iraq Campaign Medal; and

	c.  service in Iraq from 6 December 2010 to 30 October 2011.

6.  Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom (OIF).  The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to 31 December 2011.  A bronze service star is authorized for wear with this medal for participation in each credited campaign.  Approved campaigns include New Dawn (1 September 2010 - 31 December 2011)

DISCUSSION AND CONCLUSIONS:

1.  The available evidence clearly supports the applicant's contentions that she served in Iraq.  Furthermore, there is no evidence showing she had served in Afghanistan.  Therefore, it would be appropriate to remove the Afghanistan Campaign Medal with campaign star from her DD Form 214 and replace it with the Iraq Campaign Medal with one bronze service star for her service during Operation New Dawn.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  removing the Afghanistan Campaign Medal with Campaign Star from her DD Form 214 ending on 14 December 2011; and

	b.  adding the Iraq Campaign Medal with one bronze service star to this same DD Form 214.
