THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show she earned one bronze service star instead of two with her Iraq Campaign Medal (ICM).
2. The applicant states:
a. she served in Iraq from 28 April through 2 November 2009;
b. although her unit participated in two campaigns during Operation Iraqi Freedom (OIF), she deployed at a later time and only participated in one campaign.
3. The applicant provides:
* DD Form 214
* DD Form 215 (Correction to the DD Form 214)
CONSIDERATION OF EVIDENCE:
1. The applicant was appointed a Reserve commissioned officer in the rank of second lieutenant/O-1 and entered active duty in the Regular Army on 5 September 2008. She was trained in and held area of concentration 74A (Chemical, Biological, Radiological, and Nuclear Officer).
2. Her record shows she was awarded the Army Achievement Medal (AAM) for meritorious service in support of OIF during the period 28 April - 31 October 2009. On 15 December 2012, she was honorably discharged from active duty in the rank of captain/O-3, having completed 4 years, 3 months, and 11 days of creditable active duty service.
3. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of her DD Form 214 includes an entry which shows she earned the ICM with 2 bronze service stars. Item 18 (Remarks) includes the entry "SERVICE IN IRAQ 20090428 - 20091102."
4. Army Regulation 600-8-22 (Military Awards) states the ICM is awarded to members who served in direct support of Operation Iraqi Freedom. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The ICM period of eligibility is on or after 19 March 2003 through 31 December 2011. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-31 August 2010)
* New Dawn (1 September 2010-31 December 2011)
5. Army Regulation 635-5 (Separation Documents) at the time prescribed the separation documents prepared for individuals upon retirement, discharge, or release from active military service or control of the Army. It also establishes standardized policy for preparing and distributing the DD Form 214. It states for item 13, enter all awards and decorations for all periods of service and each entry will be verified by the Soldier's records.
DISCUSSION AND CONCLUSIONS:
An AAM certificate and the item 18 entry contained on her DD Form 214 corroborate the applicant's claim of service in support of OIF from on 28 April through 2 November 2009. Based on this service, she only participated in one campaign during her service in support of OIF. Therefore, item 13 of her DD Form 214 should be corrected at this time to show she earned one bronze service star with her ICM.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending item 13 of her DD Form 214 by deleting the Iraq Campaign Medal with two bronze service stars and replacing it with the Iraq Campaign Medal with one bronze service star.
