THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests change of item 19a (Mailing Address after Separation) of his DD Form 214 to show an address in South Point, OH.

2.  The applicant states the address shown was incorrectly submitted by an Army clerk.

3.  The applicant provides a copy of his DD Form 214.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 16 July 1996.  He completed training and he was awarded military occupational specialty 12B (combat engineer).  He served through continuous reenlistments.  The highest rank/grade he attained while serving on active duty was staff sergeant (SSG)/E-6.

2.  On 9 December 2011, he was honorably discharged.  He completed 15 years, 4 months, and 24 days of creditable active service.

3.  Item 19a of his DD Form 214 shows a Hardeeville, SC, mailing address.

4.  Item 21a (Member Signature) of his DD Form 214 shows he electronically signed and authenticated the form on 9 December 2011.


5.  Army Regulation 635-5 (Personnel Separations - Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It further states that the mailing address after separation and nearest relative will be provided by the Soldier.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request to have his DD Form 214 changed to show a South Point, OH, address was carefully considered.

2.  Although the applicant states the address was incorrectly entered on his DD Form 214, it appears he provided the address as shown on his DD Form 214 that he electronically signed on 9 December 2011 indicating it was correct at the time.

3.  In view of the foregoing, there is no basis for granting the relief he requests.
