THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal and correction of item 19a (Mailing Address After Separation) to show a street address of "Tradewinds" instead of "Windtree." 2. The applicant states that the Army Commendation Medal is not represented in item 13 on his DD Form 214 and his mailing address is incorrect. 3. The applicant provides a copy of his DD Form 214; a DA Form 638 (Recommendation for Award); and an award certificate for the Army Commendation Medal in support of his application. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 14 June 2004, served as a signal support systems specialist, and was released from active duty on 13 July 2008. 2. Item 13 on the applicant's DD Form 214 does not show the Army Commendation Medal as an authorized award. Item 19a on his DD Form 214 shows Windtree Drive as his street address. 3. The applicant provided an award certificate for the Army Commendation Medal for meritorious service during the period 24 March 2005 to 11 July 2008. This certificate also shows that this award was authorized by Headquarters, 18th Fires Brigade (Airborne) Permanent Order Number 163-001, dated 12 June 2008. 4. A review of the applicant’s Official Military Personnel File on the interactive Personnel Electronic Records Management System (iPERMS) revealed a copy of his residential lease agreement for the period 1 December 2008 to 30 November 2009. This contract shows he resided at the Windtree Apartments on Tradewinds Drive. 5. The applicant's address shown on his application is the same address on his rental contract. DISCUSSION AND CONCLUSIONS: 1. Orders show the applicant received the Army Commendation Medal. Therefore, his DD Form 214 should be corrected to show this medal. 2. Item 19a on the applicant's DD Form 214 shows a street address on Windtree Drive. However, the applicant contends that the correct mailing address after separation is Tradewinds Drive and his residential lease agreement shows he resided at the Windtree Apartments on Tradewinds Drive. In addition, the applicant's address shown on his application is the same address on his rental contract. Therefore, it would be appropriate to amend item 19a on his DD Form 214.
