Application for correction of military records (with supporting documents provided, if any). 2. Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Achievement Medal. She also requests the Technical Transportation of Hazardous Materials, Nuclear Biological and Chemical (NBC) Defense, and Primary Leadership Development Courses be added to item 14 (Military Education) of her DD Form 214. 2. The applicant states the certificates were not available at the time of her discharge. 3. The applicant provides her DD Form 214, Army Achievement Medal Certificate, and three certificates of training. CONSIDERATION OF EVIDENCE: 1. The applicant's military records show she enlisted in the U.S. Army Reserve on 8 April 2003 and served until she enlisted in the Regular Army on 15 November 2006. She served in military occupational specialty 92L (Petroleum Laboratory Specialist). She was promoted to pay grade E-5. 2. Permanent Orders Number 07-251-02 issued by the 4211th U.S. Army Hospital awarded her the Army Achievement Medal on 8 September 2007. 3. The applicant's records maintained in the interactive Personnel Electronic Records Management System contain a DA Form 1059 (Service School Academic Evaluation Report) which shows she attended the Primary Leadership Development Course during the period 7 to 21 May 2005. 4. She provided: * a Certificate of Training showing she successfully completed a 2-week NBC Defense Course during the period 5 to 19 September 2007 * a Certificate of Training showing she successfully completed a 2-week Technical Transportation of Hazardous Materials Course during the period 5 to 15 January 2010 5. She was honorably discharged from active duty on 7 May 2010. She completed 3 years, 5 months, and 23 days of creditable active service. Her DD Form 214 does not show the Army Achievement Medal or the Noncommissioned Officer (NCO) Professional Development Ribbon 6. Army Regulation 600-8-22 (Military Awards) states the NCO Professional Development Ribbon was established by the Secretary of the Army on 10 April 1981. It is awarded to Active Army, Army National Guard, and U.S. Army Reserve Soldiers for successful completion of designated NCO professional development courses. All Active Army, Army National Guard and Army Reserve Soldiers in an active status are eligible for award of the basic ribbon for satisfactory completion of the Primary Leadership Development Course. 7. Army Regulation 635-5 (Separation Documents) states that military education will be taken from the Enlisted Record Brief and will list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214. The entry will include the course title, length in weeks, and year completed. DISCUSSION AND CONCLUSIONS: 1. Permanent orders awarded the applicant the Army Achievement Medal. As such, she is entitled have this award added to her DD Form 214. 2. Her records show she attended the Primary Leadership Development Course prior to the period of active duty covered by her DD Form 214. As such, she is not entitled to have this course entered on her DD Form 214. However, she is entitled to have the NCO Professional Development Ribbon added to her DD Form 214. 3. The Certificate of Training provided by the applicant shows she successfully completed a 2-week NBC Defense Course during the period 5 to 19 September 2007. Therefore, she is entitled to have item 14 of her DD Form 214 corrected to show the "NBC Defense Course, 2 weeks, 2007." 4. The Certificate of Training provided by the applicant shows she successfully completed a 2-week Technical Transportation of Hazardous Materials Course during the period 5 to 15 January 2010. Therefore, she is entitled to have item 14 of her DD Form 214 corrected to show the "Technical Transportation of Hazardous Materials Course, 2 weeks, 2010." 5. In view of the foregoing, the applicant is entitled to have her records corrected as shown below. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. adding the Army Achievement Medal and the basic NCO Professional Development Ribbon to item 13 of her DD Form 214; and b. adding the following to item 14 of her DD Form 214: (1) "NBC Defense Course, 2 weeks, 2007"; and (2) "Technical Transportation of Hazardous Materials Course, 2 weeks, 2010." 2. The Board further determined that the evidence presented was insufficient to warrant a portion of the requested relief.
