THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show she completed basic combat training (BCT) and advanced individual training (AIT).

2.  She states her DD Form 214 is in error because she actually graduated from BCT and AIT.  As a result, her accomplishments should be properly reflected on her DD Form 214.

3.  She provides her:

* course completion certificates
* training record
* military schooling transcripts 
* various orders
* National Guard Bureau (NGB) Form 22 (Report of Separation and Record of Service)
* Enlisted Record Brief (ERB)

CONSIDERATION OF EVIDENCE:

1.  Her military personnel record shows she enlisted in the North Carolina Army National Guard (NCARNG) on 9 October 2009.

2.  She provides a course completion certificate and release from attachment orders which show she completed BCT at Fort Sill, OK, on 2 April 2010 and would proceed to Fort Leonard Wood, MO, on or about 3 April 2010 to attend AIT.

3.  A Fort Leonard Wood Form 655 (Department of the Army Certificate of Training) shows she successfully completed AIT for military occupational specialty (MOS) 88M (Motor Transport Operator) on 4 June 2010.

4.  NCARNG Orders 202-859, dated 21 July 2010, show she was awarded MOS 88M1O effective 4 June 2010.

5.  The applicant's records contain a DA Form 4856 (Developmental Counseling Form), dated 27 July 2010.  This document shows she was counseled on a recommendation to be discharged from the Army for her inability to complete all her AIT graduation requirements.  After several attempts, she failed to pass her Army Physical Fitness Test (APFT) with a minimum score of 60 points in each event.

6.  Section VI (Military Education) of her ERB is blank.

7.  Her command initiated a separation action under the provisions of Army Regulation 635-200 (Active Duty Enlisted Administrative Separations), paragraph 5-17, for other designated physical or mental conditions.  Her chain of command endorsed the action and recommended an honorable discharge.

8.  Her DD Form 214 shows she was honorably released from active duty and transferred to the NCARNG on 8 September 2010.  She completed 7 months and 20 days of net active service and 3 months and 10 days prior inactive service.  Item 11 (Primary Specialty) contains the entry "None//Nothing Follows" and item 14 (Military Education) contains the same.  Item 18 (Remarks) shows she did not complete her first full term on service.

9.  She also provided a copy of an NGB Form 22 which shows she was discharged from the NCARNG on 9 September 2010.  Item 12 (Military Education) of this form shows the entry "88M1OYY Motor Transport Operator" and item 13 (Primary Specialty Number, Title, and Date Awarded) contains the same entry as item 12.

10.  Army Regulation 635-5 (Separation Documents) prescribes policies and procedures regarding separation documents.  Paragraph 2-4 (Completing the DD Form 214) provides detailed instructions and source document(s) for completing each block of the DD Form 214.  It states to list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by the DD Form 214 in item 14 from the ERB.  Include title, length in weeks, and year completed.  This information is to assist the Soldier in job placement and counseling; therefore, do not list training courses for combat skills.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's request to add BCT and AIT to her DD Form 214 was carefully considered; however, it is not supported by the evidence of record.

2.  Although MOS orders and a certificate were printed showing she completed all the academic requirements for award of MOS 88M, she failed to pass the required APFT by scoring a minimum of 60 points in each event.  As a result, she did not meet all the requirements to be awarded MOS 88M.

3.  Army Regulation 635-5 states that training courses for combat skills will not be listed on the DD Form 214.  The training the applicant requests to be added to her discharge document are either training courses for combat skills (BCT) or training courses (AIT) where all requirements for successful completion were not met.  Therefore, it is not appropriate to list these training courses on the applicant's DD Form 214.
