The Board considered the following evidence:
Exhibit A - Application for correction of military records.
Exhibit B - Military Personnel Records (including advisory opinion,
if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. In effect, the applicant requests that a DD Form 215 be issued to
correct his DD Form 214 (Certificate of Release or Discharge from Active
Duty).
2. The applicant states that his DD Form 214 should be corrected to show
his primary specialty as 91W3HM6, vice 91W30 [item 11 of his DD Form
214], and to show that the title of the course that he completed in 1987
as Health Care Specialist (Licensed Practical Nurse) Phases I & II, 54
weeks, 1987, vice Health Care Specialist Phases I & II, 54 weeks, 1987
[item 14 of his DD Form 214].
3. The applicant provides the evidence indicated herein.
CONSIDERATION OF EVIDENCE:
1. The applicant's DD Form 214 shows that he retired from the Army on
31 December 2003 with more than 20 years of active federal service.
His primary specialty shown on that form is 91W30 Health Care Specialist –
19 years – 8 months. One of the courses shown that he completed is Health
Care Specialist Phases I & II, 54 weeks, 1987.
2. Submitted with his request are documents which purport to show the
military courses, to include the MOS (military occupational specialty)
training courses that he completed during his military career, among which
are the Primary Leadership Development Course (PLDC), Clinical Specialist
Course, Drill Sergeant School, Medical Specialist Courses (MOS 91A and
91B), Practical Nurse courses (MOS 91C), and Instructor (Special
Qualification Identifier H).
3. A Service School Academic Evaluation Report shows that the applicant
completed a resident Practical Nurse Course in September 1988. A diploma
issued by the Academy of Health Sciences also indicates that he completed
that course.
4. A document, titled "National Council Licensure Examination for
Practical Nurses," dated 18 October 1988, indicates that he passed [the
examination].
5. On 29 August 1988 the applicant was awarded the primary MOS of 91C10,
effective on 23 September 1988, and that his previous primary MOS of 91A10
was withdrawn.
6. An order published by the Walter Reed Army Medical Center on 26 June
1997 shows that he was awarded the primary MOS of 91C3H and that his
primary MOS of 91C30 was withdrawn.
7. Certificates show that the applicant completed a faculty development
course, phase I, from 21-25 April 1997.
8. Army Regulation 635-5 provides instructions for the preparation of the
DD Form 214, and states in effect that information for completion of
item 11 (primary specialty) will be obtained from the ERB (enlisted record
brief), entering the titles of all MOS served for at least 1 year and
include for each MOS the number of years and months served. Specify the
first 5 characters of the primary MOS code, which includes the 3 characters
of the MOS, the fourth character of skill and grade level in the MOS, and
the fifth character of a special qualification identifier (SQI), if
applicable.
9. Information for completion of item 14 (military education) will be
obtained from the ERB, listing formal in-service (full-time attendance)
training courses successfully completed, to include title, length in weeks,
and year completed.
10. Information obtained from an official of the Health Services Branch,
Enlisted Personnel Directorate of the Human Resources Command, on 20 July
2004 revealed that the MOS 91B (Medical Specialist) and 91C (Practical
Nurse), were changed in 2001 to 91W00 and 91W00M6, respectively.
DISCUSSION AND CONCLUSIONS:
1. The applicant served as a practical nurse since at least 1997, when
orders published by the Walter Reed Army Medical Center, show that his MOS
was changed to identify him as an instructor (SQI "H," the fifth digit in
his MOS code). His MOS, 91C3H, the "3" fourth digit identifying his skill
and grade level, should have been changed to MOS 91W3HM6, and identified as
such on his DD Form 214 with the number of years and months
served in that MOS, and in other specialties in which he served; however,
there is no such evidence and the applicant has not provided any.
Obviously, he did not serve as a practical nurse for over 19 years, but
likely was first trained in the medical field as a medical specialist, as
he himself has indicated. Because of the lack of evidence, his request to
correct item 11 of his DD Form 214, although erroneous, cannot be granted.
2. The applicant completed a Practical Nurse Course in 1988 as indicated
by the documents that he submits. The title of that course should be
indicated in item 14 in lieu of the title, Health Care Specialist Phase I &
II, and the year completed 1988, vice 1987. The applicant was apparently
licensed after he passed an examination. Nonetheless, the title of the
course is "Practical Nurse Course," not "Licensed Practical Nurse."
Consequently, his request to show that he completed a licensed practical
nurse course is not granted.ea

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to
warrant a recommendation for partial relief. As a result, the Board
recommends that all Department of the Army records of the individual
concerned be corrected by showing it item 14 of his 31 December 2003 DD
Form214 that he completed a Practical Nurse Course, 54 weeks, 1988, vice
the entry, Health Care Specialist Phases I & II, 54 weeks, 1987.
2. The Board further determined that the evidence presented is
insufficient to warrant a portion of the requested relief. As a result,
the Board recommends denial of so much of the application that pertains to
correction of item 11 of his DD Form 214 to show his primary specialty as
91W3HM6, without evidence as to the time that he served in that specialty.
The Board also recommends denial of so much of the application that
pertains to correction of item 14 of his DD Form 214 to show that he
completed a course titled, Health Care Specialist (Licensed Practical
Nurse) Phases I & II, 54 weeks, 1987.

The same regulation states to list in-service (full-time attendance) training courses successfully completed during the period of service in item 14 of the DD Form 214. Her request to have MOS 91C added to item 11 and the Practical Nurse Course to item 14 of her DD Form 214 was carefully considered. Both conversion periods were prior to the applicants retirement from the Army and accordingly, her MOS was converted during each period.
