THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests the removal of a duplicate order (#312-107) from his Official Military Personnel File (OMPF).

2.  The applicant states that order #312-107 is filed twice on the performance section of his OMPF and he desires one copy to be removed.

3.  The applicant provides a copy of his Army Commendation Medal (ARCOM) Certificate issued under Permanent Order #312-107.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army in Richmond, Virginia on 
9 January 2003 for a period of 3 years, airborne training, training as a fire support specialist and a seasonal cash enlistment bonus.  He completed his training and has remained on active duty through continuous reenlistments.  He was promoted to the pay grade of E-6 on 1 September 2007.

2.  Permanent Order #312-107 issued by Headquarters, 2d Brigade Combat Team, 25th Infantry Division on 7 November 2004 in Kirkuk, Iraq awarded the applicant the ARCOM for meritorious service from 21 January 2004 to 1 January 2005.  The order was filed in the performance section of his OMPF on 7 November 2004 and again on 1 January 2005.



DISCUSSION AND CONCLUSIONS:

1.  The applicant’s contention has been noted and found to have merit.  His OMPF contains two copies of permanent orders #312-107 filed in the performance section of his OMPF.

2.  Accordingly, one copy of permanent orders #312-107 should be deleted/removed from his OMPF.

BOARD DETERMINATION/RECOMMENDATION:

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by deleting one copy of permanent order #312-107 from the performance section of his OMPF.
