      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, removal of an Army Achievement Medal
(AAM) certificate and the DA Form 638 (Recommendation for Award), Permanent
Orders Number 298-02 dated October 2002.

2.  The applicant states the AAM was originally supposed to be the award of
the Army Commendation Medal (ARCOM).  She said she had the award amended
and a corrected copy was sent to her Official Military Personnel File
(OMPF).  However, she is currently waiting for the certificate to be added
to her records.

3.  The applicant provides a copy of the DA Form 638, Permanent Orders
Number 340-02, AAM and ARCOM certificates.

CONSIDERATION OF EVIDENCE:

1.  The applicant’s records show on 3 June 2002, she was recommended for an
ARCOM for achievement while she was assigned to B Company, United States
Army Medical Detachment, Fort Hood, Texas.  The period of award is listed
as   1 November 2001 to 15 March 2002.

2.  The DA Form 638 shows that the award was approved as an ARCOM by the
Medical Detachment Commander.  However, the orders approval authority
approved the award as an AAM.  The DA Form 638 was assigned permanent
orders number 298-02, dated 25 October 2002.

3.  The AAM certificate coincides with the DA Form 638 and shows the same
orders number and date.  The approval authority on both the DA Form 638 and
the AAM certificate are the same.  The DA Form 638 is on file on the
applicant’s OMPF but, the AAM certificate is not on file.

4.  Permanent Orders Number 340-02, dated 8 December 2005, amended the AAM
to read Award:  Army Commendation Medal (1st Oak Leaf Cluster).  An ARCOM
certificate was issued that contains the exact information as the AAM
certificate with the exception of the approval authority.  The permanent
orders are listed on the applicant’s OMPF; however, the ARCOM certificate
is not on file.






DISCUSSION AND CONCLUSIONS:

1.  Evidence of record shows the applicant was recommended and approved for
the award of the ARCOM.  However, the orders approval authority approved
the award of the AAM and issued an AAM certificate in October 2002 instead
of the ARCOM.

2.  In December 2005, this mistake was corrected and an amended order was
issued changing the approved AAM award to its intended award of the ARCOM.
Additionally, an ARCOM certificate was issued.  The amended order and the
DA Form 638 are both on file on the applicant’s OMPF.  However, neither the
AAM nor the ARCOM certificate is listed on her OMPF.  Therefore, in the
interest of justice, the applicant's records will be corrected to delete
the DA Form 638 and Permanent Orders Number 340-02 from the applicant’s
OMPF.  Additionally, a copy of the ARCOM certificate will be placed on her
OMPF.

3.  In the event that a copy of the AAM certificate is submitted for filing
on the applicant’s OMPF, this document will serve as authorization to
delete that document prior to filling.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected as
follows:

     a. by deleting DA Form 638, dated 25 October 2002;

      b.  by deleting Permanent Orders Number 340-02, dated
6 December 2005; and

      c.  by adding Army Commendation Medal certificate from
1 November 2001 to 15 March 2002.
