THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show his foreign service and deployments.
2. He states his DD Form 214 shows he had no foreign service or deployments which is false.
3. He provides his DD Form 214, orders authorizing him to wear former wartime service shoulder sleeve insignia for his service in support of Operation Iraqi Freedom and a memorandum authorizing him to wear the Iraq Campaign Medal.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 25 March 2005. He was honorably released from active duty on 4 December 2009 after completing 4 years, 8 months, and 10 days of net active service.
2. His DD Form 214 shows no period of foreign service in item 12f (Foreign Service) and does not show he was deployed during his active duty service.
3. His Enlisted Record Brief (ERB) shows he was deployed to Iraq during the following periods:
* 28 March to 12 July 2006
* 4 October 2007 to 9 January 2008
* 30 June to 19 October 2008
4. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214.
a. It states for item 12f, enter the total amount of foreign service completed during the period covered in item 12c from the ERB.
b. It states that for an active duty Soldier deployed with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" in item 18 (Remarks).
DISCUSSION AND CONCLUSIONS:
1. The applicant's ERB shows he was deployed to Iraq three times: 28 March to 12 July 2006 (3 months and 15 days), 4 October 2007 to 9 January 2008 (3 months and 6 days), and 30 June to 19 October 2008 (3 months and 20 days). His total foreign service is as follows:
3 months 15 days
3 months 6 days
+ 3 months 20 days
10 months 11 days
2. He is entitled to correction of his DD Form 214 to show he had 10 months and 11 days of foreign service.
3. He is also entitled to correction of his DD Form 214 to include a statement documenting his deployed service in Iraq.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined the evidence presented is sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by making the following changes to his DD Form 214:
* delete "0000 00 00" from item 12f
* add "0000 10 11" to item 12f
* add the statement "SERVICE IN IRAQ FROM 20060328-20060712, 20071004-20080109, 20080630-20081019" to item 18
