THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, item 12f (Foreign Service) and item 18 (Remarks) of her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show her dates of foreign service during Operation Iraqi Freedom (OIF). 2. The applicant states: * her dates of service during OIF were 26 January 2003 to 12 July 2003 * her foreign service was not entered on her DD Form 214 3. The applicant provides: * Enlisted Record Brief (ERB) * page 2 of DA Form 2166-8 (Noncommissioned Officer Evaluation Report) * DD Form 214 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 18 March 1988 and trained in food service operations. On 31 March 2008, she retired in the rank of sergeant first class. 2. Item 12f of her DD Form 214 shows the entry "0000  00  00." Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) shows, among other awards, the Global War on Terrorism Expeditionary Medal and Global War on Terrorism Service Medal as authorized awards but does not show the Iraq Campaign Medal. Item 18 does not show any deployments. 3. Section I (Assignment Information/Overseas/Deployment Combat Duty) of her ERB shows the entry "20030126-20030712" [26 January 2003-12 July 2003] in Iraq for a total of 4 months and 18 days. 4. Records at the Defense Finance and Accounting Service show she received hostile fire/imminent danger pay during the period 28 January 2003 to 11 July 2003 in Kuwait. 5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It establishes standardized policy for the preparation of the DD Form 214. The regulation states, for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD-YYYYMMDD)" will be entered in item 18. 6. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who served in direct support of OIF. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of OIF. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are: * Liberation of Iraq (19 March 2003-1 May 2003) * Transition of Iraq (2 May 2003-28 June 2004) * Iraqi Governance (29 June 2004-15 December 2005) * National Resolution (16 December 2005-9 January 2007) * Iraqi Surge (10 January 2007-31 December 2008) * Iraqi Sovereignty (1 January 2009-date to be determined) DISCUSSION AND CONCLUSIONS: 1. The entry in the applicant's ERB and her confirmation of service in Kuwait/Iraq are accepted as sufficient evidence on which to amend item 12f and item 18 of her DD Form 214 to show she served in Kuwait/Iraq from 26 January 2003 to 12 July 2003 for a total of 4 months and 18 days. 2. As a related issue, based on her service in Iraq she is eligible for award of the Iraq Campaign Medal with two bronze service stars. Her DD Form 214 should be corrected to show this medal.
