THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show foreign service in Panama, Djibouti, and Iraq. 2. The applicant states he did not review his DD Form 214 well when he outprocessed. 3. The applicant provides: * Enlisted Record Brief (ERB) * Permanent Order (PO) Number 250-1, dated 7 September 1998, issued by U.S. Military Assistance and Advisory Group (MAAG), Peru * Memorandum for Record, dated 17 November 2004, issued by the 202nd Quartermaster Company (WP), subject: Orders Number 296-1, 22 October 2004 CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army on 10 April 1992. After completing initial training, he was awarded military occupational specialty (MOS) 52D (Power Generation Equipment Repairer). This MOS later changed to the 91-series (Field Mechanical Maintenance). The highest rank/grade held was sergeant first class/E-7. 2. On 30 April 2012, he retired and was placed on the Retired List. His DD Form 214 shows he completed 20 years and 21 days of total service for retired pay. Item 12f (Foreign Service) of his DD Form 214 shows no foreign service. Item 18 (Remarks) contains no entries showing service in a combat zone. 3. His ERB shows Overseas Service/Deployment Combat Duty to: * Panama (19950122-19990405) (4 years, 2 months, and 15 days shown as a long tour) * Djibouti (20020901-20021212) (3 months and 12 days listed as a deployment) * Iraq (20050113-20051213) (11 months and 1 day identified as a deployment) 4. According to the Defense Finance and Accounting Service (DFAS), the applicant received Hostile Fire/Imminent Danger Pay and Combat Zone Tax Exclusion for the period 13 January 2005 to 17 January 2006 for service in Kuwait. 5. The applicant provides: a. PO Number 250-1 showing he was on temporary duty to Peru from 14 July 1998 to 14 October 1998. b. A Memorandum for Record stating the applicant was identified to deploy in support of Operation Iraqi Freedom III with the 202nd Quartermaster Company. 6. Army Regulation 635-5 (Separation Documents), in effect at the time, prescribed policy and procedures for the completion of the DD Form 214. a. Regarding item 12f, it states the source of the entry is the ERB. b. As to entering dates of service while being deployed, the regulation states, for a Soldier deployed to a foreign country enter in item 18, "Service in (name of country deployed) from (inclusive dates) shown as YYYYMMDD - YYYYMMDD." DISCUSSION AND CONCLUSIONS: 1. There is merit to the applicant's request to correct his record to show his foreign service. Both the applicant's ERB and DFAS confirm he served overseas and his DD Form 214 should be corrected to reflect this service. a. His ERB states he served in Panama for 4 years, 2 months, and 15 days. b. His ERB also shows he served in Djibouti for the period 1 September 2002 to 12 December 2002 for a total of 3 months and 12 days. His ERB shows this as a deployment. c. Information from his ERB and DFAS affirms he deployed to Kuwait/Iraq from 13 January 2005 to 14 January 2006 for a total of 1 year and 5 days. d. Item 12f of his DD Form 214 should be amended to show a total of 5 years, 6 months, and 2 days of foreign service for his three overseas periods identified on his ERB.
