THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that block 17 (Member Was Provided Complete Dental Examination and All Appropriate Dental Services and Treatment within 90 Days Prior to Separation) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show he did not receive dental services within 90 days of separation.

2.  The applicant states he brought this error to the attention of his command at the time of separation and was told he would receive a DD Form 215 (Correction to DD Form 214 - Certificate of Release or Discharge from Active Duty).  He was told he would receive dental care through the Department of Veterans Affairs (VA) but the VA will not provide the necessary care until his records are corrected.

3.  The applicant provides copies of his DD Form 214, a 13 November 2008 Standard Form 603-A (Health - Dental Record), and his retirement orders.

CONSIDERATION OF EVIDENCE:

1.  The applicant, a career Army Reserve lieutenant colonel with over 34 years of prior active and inactive service, was called active duty in support of Operation Iraqi Freedom on 13 November 2007. 

2.  On 13 November 2008, the applicant's dental record was given a demobilization audit and the results recorded on the Standard Form 603-A show that the applicant had not received a complete dental examination and all 

appropriate dental services or treatment within 90 days prior to separation.  
X-rays were taken and he was shown to have moderate caries risk.

3.  The applicant was released from active duty on 14 January 2009 having served 1 year, 2 months, and 2 days during this mobilization.  Block 17 of the DD Form 214 is marked "yes" for receipt of dental treatment.

4.  The applicant was transferred to the Retired Reserve effective 17 March 2009.

5.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents that must be prepared for Soldiers on retirement, discharge, release from active duty service, or control of the Active Army.  It establishes standardized policy for preparing and distributing the DD Form 214.  In pertinent part it states that the entry in block 17 is to be verified by referencing the Standard Form 603-A for validity of exam and all appropriate treatment.

DISCUSSION AND CONCLUSIONS:

1.  The available Standard Form 603-A shows the applicant had not received all appropriate dental services and treatment within 90 days prior to separation.

2.  It appears that a typographical error occurred at the time of the preparation of the applicant's DD Form 214 that incorrectly shows he had received all appropriate dental services and treatment within 90 days prior to separation.

3. Therefore, it is appropriate to correct block 17 of the DD Form 214 to show "no."

As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by issuing the applicant a DD Form 215 amending block 17 of the DD Form 214 to show "no."
