THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show that he was not provided a complete dental examination and all appropriate dental services and treatment within
90 days prior to separation.
2. The applicant states that he cannot receive dental treatment from the Department of Veterans Affairs (VA) until his DD Form 214 is corrected.
3. The applicant provides, in support of his application, copies of his DD Form 214 and extracts from his service dental records.
CONSIDERATION OF EVIDENCE:
1. On 22 February 2008, the applicant, an Army National Guard (ARNG) Soldier, was ordered to active duty in support of Operation Iraqi Freedom. He entered active duty this period in the rank/grade of staff sergeant (SSG)/E-6.
2. A Standard Form (SF) 603A (Health Record-Dental-Continuation) pertaining to the applicant shows that on 10 February 2009 he was not provided a complete dental exam and he did not receive dental services or treatment prior to separation. He did receive a demobilization exam and dental x-rays.
3. The applicant served on active duty for a period of 1 year, 1 month, and
10 days. He was released from active duty on 1 April 2009 and he was transferred back to his ARNG unit.
4. Item 17 (Member was provided complete dental examination and all appropriate dental services and treatment within 90 days prior to separation) of his DD Form 214 for the period ending 1 April 2009 shows an "x" under YES.
5. Army Regulation 635-5 (Separation Documents) provides that the DD Form 214 is a summary of a Soldier's most recent period of continuous active duty. It provides a brief, clear-cut record of active duty service at the time of release from active duty, retirement, or discharge. It further provides that Item 17 applies only to Soldiers who have completed 180 days or more of continuous active duty. The source document used for validating the entry in Item 17 is the Soldiers
SF 603 (Health Record-Dental) for validity of examination and all appropriate treatment.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that his DD Form 214 should be corrected to show that he did not receive a complete dental examination or treatment within 90 days prior to his separation.
2. The applicant's SF 603A, the designated source document, clearly shows that the applicant did not receive a complete dental examination or treatment within 90 days prior to his separation. Accordingly, his DD Form 214 should be corrected to show an "x" under "NO."

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. deleting from Item 17 of his DD Form 214 for the period ending 1 April 2009 the "x" under "YES"; and
b. adding to Item 17 of his DD Form 214 for the period ending 1 April 2009 an "x" under "NO."
