THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her DD Form 214 dated 29 March 2008 be corrected to reflect a "NO" entry in block 17 under "Member was provided complete dental examination and all appropriate dental services and treatment within 90 days prior to separation".

2.  The applicant states, in effect, that she had a root canal performed about 1 week before she began clearing for separation and it required a crown to be placed on the tooth afterwards.  However, due to the limited amount of time left before her expiration of term of service (ETS) date it could not be accomplished and she now desires to apply for treatment at the Department of Veterans Affairs (VA) to complete the treatment.

3.  The applicant provides a copy of her DD Form 214, a memorandum explaining her application and a memorandum from the attending dentist at the time.

CONSIDERATION OF EVIDENCE:

1.  The applicant initially enlisted in the Regular Army on 21 September 1994 and served until she was honorably released from active duty on 8 June 2001, due to completion of required service.

2.  She again enlisted in the Regular Army with a waiver on 2 March 2002 and served until she was honorably discharged at Fort Irwin, California in the pay grade of E-6 on 29 March 2008, due to completion of required service.  She had served 12 years, 8 months and 28 days of total active service and her DD Form 214 reflects that she was provided a complete dental examination and all appropriate dental services and treatment within 90 days prior to separation.

3.  The memorandum provided by the applicant with her application from her attending dentist, an Army captain at Fort Irwin, California, indicates that he performed a root canal on tooth #15 and that it needed a full coverage crown.  However, due to time constraints, that service was not accomplished before the applicant left Fort Irwin.

4.  Army Regulation 635-5 (Personnel Separations - Separation Documents) serves as the authority for the preparation of the DD Form 214.  It provides, in pertinent part, that the Standard Form (SF) 603 (Health Record – Dental) will be reviewed to determine if the appropriate exams and all appropriate treatment has been accomplished prior to separation.

DISCUSSION AND CONCLUSIONS:

1.  The statement from the attending dentist clearly indicates the applicant had been identified for further dental treatment prior to separation at Fort Irwin, California.

2.  Therefore, it is reasonable to presume that an administrative oversight resulted in the "Yes" block being completed in item 17 of her DD Form 214 at the time of discharge.

3.  Accordingly, it would be in the interest of justice to correct that oversight at this time by showing that she did not receive all appropriate treatment prior to discharge by changing the "Yes" in block 17 to a "No".

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by changing block 17 of her DD Form 214 to reflect a "NO" instead of the "Yes" entry that is currently reflected.
