THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that block 17 (Member was Provided Complete Dental Examination and all Appropriate Dental Service and Treatment within 90 Days Prior to Separation) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show "No" instead of "Yes." 2. The applicant states his dental treatment was pending, it was not completed. He states he is in need of urgent dental care due to pain and discomfort. 3. The applicant provides a letter addressed to him from the National Personnel Records Center, dated 28 August 2009. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the Regular Army for 3 years, and 17 weeks on 5 May 2004. He completed training as an armor crewman. He extended his enlistment for 4 months on 15 October 2004. 2. Due to essential national security, the applicant was retained in the Army for 522 days. 3. On 1 June 2009, the Branch Chief, Personnel Processing at Headquarters, United States Army Garrison, Fort Carson, Colorado, forwarded a memorandum to the Commander, Soldier Record Data Center, requesting that the applicant be issued a DD Form 215 (Correction to DD Form 214) that changing block 17 to show "No" instead of "Yes." According to the Branch Chief, the required change was due to an administrative error. 4. The applicant was honorably released from active duty on 5 June 2009 at the completion of his required active service. Block 17 on his DD Form 214 states "Member was Provided Complete Dental Examination and all Appropriate Dental Service and Treatment within 90 Days Prior to Separation" and the block is marked "Yes" instead of "No." 5. Army Regulation 635-5 (Separation Documents) states that block 17 is applicable only to Soldiers who have completed 180 days or more of continuous active duty. Check the Standard Form (SF) 603 (Health Record-Dental) for validity of exam and all appropriate treatment). Enter "NA" for Soldiers who had fewer than 180 days of continuous active duty. 6. There is no SF 603 contained in the applicant's official military record. DISCUSSION AND CONCLUSIONS: 1. The applicant contends block 17 on his DD Form 214 should be corrected to show "No" instead of "Yes." 2. There is no SF 603 contained in the available record; however, the memorandum forwarded to the Commander, Soldier Record Data Center by the Branch Chief, Personnel Processing clearly explains that an administrative error was made which resulted in block 17 being marked "Yes" instead of "No." The Branch Chief, Personnel Processing also requested that a DD Form 215 be prepared to correct the error on the applicant's DD Form 214. 3. There is no evidence contained in his records that shows a DD Form 215 was prepared to correct block 17 on his DD Form 214. 4. It would be in the interest of justice to correct the applicant's records as recommended below.
