THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of the date of graduation from the Special Forces Qualification Course.  

2.  The applicant states his graduation date from the Special Forces Qualification Course is incorrect.  He contends the clerk placed the incorrect date on his DD Form 214 (Certificate of Release or Discharge from Active Duty).  

3.  The applicant provides a copy of his DD Form 214; his diploma from the Special Forces Qualification Course, dated 12 April 1985; and his DA Form 1059 (Service School Academic Evaluation Report), dated 12 April 1985, in support of his application.  

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 17 November 1983.  He completed the required training and was awarded military occupational specialty 05C (Radio Operator).  His highest grade attained was master sergeant, E-8.  

2.  The applicant’s DA Form 2-1 (Personnel Qualification Record - Part II) shows in September 1984 he was assigned to Fort Bragg, NC for completion of the Special Forces Qualification Course.  Item 17 (Civilian and Military Schools) of his DA Form 2-1 shows he completed 20 weeks of training in the Special Forces Qualification Course in 1985.  

3.  The applicant provided a diploma, dated 12 April 1985, from the U.S. Army John F. Kennedy Special Warfare Center (USAJFKSWC), which shows he completed the Special Operations Communications Sergeant Course during the period 1 October 1984 to 12 April 1985.

4.  His DA Form 1059, dated 12 April 1985, shows he completed the Special Operations Communications Sergeant Course on 12 April 1985.  

5.  The applicant was retired from active duty on 30 November 2008 under the provisions of Army Regulation 635-200, chapter 12 based on sufficient service for retirement.

6.  The applicant’s DD Form 214, Item 18 (Remarks) shows, in part, the entry "Special Forces Qualification Course, 6 Weeks, 1999."

7.  Army Regulation 635-5 (Separation Documents) states, in pertinent part, that item 14 (Military Education) will list formal in-service training courses of 40 hours or more successfully completed during the period of service covered by title, length in weeks, and month and year completed.  This information is to assist the Soldier after separation in job placement and counseling; therefore, training courses for combat skills are not listed.

DISCUSSION AND CONCLUSIONS:

1.  The evidence of record shows the applicant completed the Special Forces Qualification Course during the period 1 October 1984 through 12 April 1985.  However, his DD Form 214 erroneously reflects that he completed the Special Forces Qualification Course in 1999.  

2.  The applicant's DA Form 2-1 confirms that he completed 20 weeks of training in the Special Forces Qualification Course in 1985.  Therefore, his DD Form 214 should be amended to show the entry "Special Forces Qualification Course, 20 Weeks, 1985."

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the entry "Special Forces Qualification Course, 6 Weeks, 1999" from his DD Form 214; and

	b.  adding the entry "Special Forces Qualification Course, 20 Weeks, 1985" to his DD Form 214.
