THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of item 14 (Military Education) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show he completed the Primary Leadership Development Course (PLDC).

2.  The applicant states that he successfully completed the PLDC and received his diploma on 2 July 2003 at Fort Bliss, Texas.

3.  The applicant provides a diploma for the PLDC, a copy of his DD Form 214, a VA Form 21-4138 (Department of Veterans Statement in Support of Claim), and a DA Form 1059 (Service School Academic Evaluation Report).

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the Regular Army on 13 September 2000, trained as a patriot operator/maintainer, and was discharged on 13 June 2008.

2.  Item 14 on the applicant’s DD Form 214 shows the entry, "NONE//NOTHING FOLLOWS."

3.  The applicant provided a DA Form 1059, dated 7 February 2003, which shows he passed the PLDC (duration of the course from 9 January 2003 to 7 February 2003).  He also provided a diploma, dated 7 February 2003, which shows he graduated from the PLDC.  A copy of this DA Form 1059 is filed on the Integrated Personnel Electronic Records Management System (IPERMS).

4.  Army Regulation 635-5 establishes the standardized policy for preparing and distributing the DD Form 214.  The regulation provides that item 14 will list formal in-service (full-time attendance) training courses successfully completed during the period of service covered by title, length in weeks, and year completed.

DISCUSSION AND CONCLUSIONS:

The DA Form 1059 and the diploma provided by the applicant which show he graduated from the PLDC on 7 February 2003 are accepted as sufficient evidence on which to amend item 14 on his DD Form 214.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting the entry in item 14 on his DD Form 214; and 

	b.  adding the entry, "Primary Leadership Development Course, 4 weeks, 2003" in item 14 on his DD Form 214.
