THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, that the location and period of his foreign service be included on his DD Form 214 (Certificate of Release or Discharge from Active Duty).  

2.  The applicant states he needs the specific dates listed to obtain benefits related to his combat service.  He believes he arrived in Kuwait on 21 August 2007 and in Iraq on 10 October 2007.

3.  The applicant provides a copy of his DD Form 214 in support of his application.

CONSIDERATION OF EVIDENCE:

1.  The applicant entered active duty on 1 June 2006, completed training, and was awarded military occupational specialty (MOS) 11B (Infantryman). 

2.  The applicant's iPERMS (interactive Personnel Electronic Records Management System) records do not contain any significant information related to the location of his foreign service or to the dates he was deployed.

3.  The applicant was discharged on 5 June 2008 with a general, under honorable conditions discharge.

4.  Block 12f (Foreign Service) on his DD Form 214 is blank and block 18 (Remarks) does not contain any entry concerning deployment to a foreign country.

5.  Available medical records show the applicant incurred a self-inflicted wound on 15 October 2007 in Iraq and was evacuated to the United States on 19 October 2007.

6.  Information obtained from the Defense Finance and Accounting Service (DFAS) shows that the applicant received Imminent Danger Pay (IDP) for the period of service in Kuwait from 1 August 2007 through 31 January 2008.

7.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It states that:

   a.  for Block 12f, from Soldier's records, enter the total amount of foreign service completed during the period covered in block 12c (Net Active Service This Period; and
   
	b.  for an active duty Soldier deployed with his or her unit during their continuous period of active service, the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)" will be entered in block 18 (Remarks).

DISCUSSION AND CONCLUSIONS:

1.  The available information shows that the applicant most likely served in Kuwait/Iraq from 21 August 2007 through 19 October 2007. 

2.  Therefore, it is appropriate to correct the applicant's DD Form 214 at block 12f to show that he had 1 month and 28 days of foreign service, and block 18 to show the entry "SERVICE IN KUWAIT/IRAQ FROM 20070821 – 20071019."

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by amending block 12f of his DD Form 214 to read "00  01  28" and at Block 18 to include the statement "SERVICE IN KUWAIT/IRAQ FROM 20070821 – 20071019."
