THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:
1. Application for correction of military records (with supporting documents provided, if any).
2. Military Personnel Records and advisory opinions (if any).
THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show the correct period of his service in Iraq and to show his overseas service.
2. The applicant states he was issued a DD Form 215 (Correction to DD Form 214) that shows incorrect dates for his service in Iraq, and his overseas service is not recorded in his military records.
3. The applicant provides a DD Form 214 and a DD Form 215.
CONSIDERATION OF EVIDENCE:
1. The applicant enlisted in the Regular Army on 8 September 2004 and was awarded military occupational specialty 21C (Bridge Crewmember) upon completion of initial entry training.
2. The Defense Finance and Accounting Service (DFAS) confirmed the applicant received hostile fire/imminent danger pay (HF/IDP) and combat zone tax exclusion (CZTE) for service in Kuwait from 6 July 2006 through 2 July 2007
(11 months and 27 days).
3. He was discharged on 19 June 2008. Block 12f (Foreign Service) of his DD Form 214 is blank. Block 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) shows the National Defense Service Medal, Global War on Terrorism Service Medal, and the Army Service Ribbon. His service in Iraq is not shown on his DD Form 214.
4. He was issued a DD Form 215, dated 9 November 2009, which corrected his DD Form 214 by amending block 12f to show 11 months and 27 days, block
13 to show the Iraq Campaign Medal, and block 18 (Remarks) to show he served in Iraq form 6 July 2007 to 2 July 2007.
5. Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army. It provides standardized policy for the preparation of the DD Form 214 and states:
a. in block 12f, enter the total amount of foreign service completed during the period covered in block 12c (Net Service this Period);
b. in block 13, list awards and decorations for all periods of service; and
c. in block 18, for an active duty Soldier deployed to a foreign country with his or her unit during their continuous period of active service, enter the statement "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)."
6. Army Regulation 600-8-22 (Military Awards) states the Iraq Campaign Medal is awarded to members who have served in direct support of Operation Iraqi Freedom. The area of eligibility encompasses all the land area of the country of Iraq, the contiguous water area out to 12 nautical miles, and all air spaces above the land area of Iraq and above the contiguous water area out to 12 nautical miles. The Iraq Campaign Medal period of eligibility is on or after 19 March 2003 to a future date to be determined by the Secretary of Defense or the cessation of Operation Iraqi Freedom. A bronze service star is authorized for wear with this medal for participation in each credited campaign. Approved campaigns are:
* Liberation of Iraq (19 March 2003-1 May 2003)
* Transition of Iraq (2 May 2003-28 June 2004)
* Iraqi Governance (29 June 2004-15 December 2005)
* National Resolution (16 December 2005-9 January 2007)
* Iraqi Surge (10 January 2007-31 December 2008)
* Iraqi Sovereignty (1 January 2009-date to be determined)
DISCUSSION AND CONCLUSIONS:
1. The applicant's contention that his DD Form 214 should be corrected to show the correct dates of his service in Iraq and to show his overseas service has been carefully reviewed.
2. DFAS records confirm he received HF/IDP and CZTE from 6 July 2006 through 2 July 2007. He was issued a DD Form 215 which corrected his DD Form 214 to show his service in Iraq; however, the DD Form 215 erroneously shows the beginning date of his service in Iraq as 6 July 2007 instead of 6 July 2006. Therefore, he should be issued a DD Form 215 showing the correct dates of his service in Iraq.
3. The DD Form 215 also corrected his DD Form 214 to show the Iraq Campaign Medal; however, he participated in two campaigns during his service in Iraq. Therefore, his records should be further corrected to show the Iraq Campaign Medal with two bronze service stars.
4. The DD Form 215 correctly shows he served 11 months and 27 days of foreign service. However, because of the error and missing information on the DD Form 215, this form should be voided and a new DD Form 215 should be issued showing the above discussed corrections.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:
a. voiding his DD Form 215 dated 9 November 2009;
b. correcting his DD Form 214 by amending block 12f to show 11 months and 27 days, block 13 to show the Iraq Campaign Medal with two bronze service stars, and block 18 to show the entry "SERVICE IN KUWAIT/IRAQ FROM 20060706- 20070702."
