THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) with the period ending 9 September 2006 to show award of the Army Commendation Medal.
2. The applicant states, in effect, that she received the above award after she was discharged from the military.
3. The applicant provides a DA Form 638 (Recommendation for Award), dated 14 September 2006, and Madigan Army Medical Center Permanent Orders 289-007, dated 16 October 2006.
CONSIDERATION OF EVIDENCE:
1. The applicant was appointed as a Reserve Commissioned Officer of the U.S. Army on 2 August 2002 and entered active duty on 5 September 2002.
2. On 9 September 2006, the applicant was honorably discharged after completing 4 years and 5 days of creditable active service.
3. Item 13 (Decorations, Medals, Badges, Citations, and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 with the period ending September 2006 does not show award of the Army Commendation Medal.
4. Madigan Army Medical Center Permanent Orders 289-007, dated 16 October 2006, show that the applicant was awarded the Army Commendation Medal for meritorious service during the period 24 December 2003 to 23 August 2006.
DISCUSSION AND CONCLUSIONS:
Evidence of record shows that the applicant was awarded the Army Commendation Medal. His DD Form 214 should be corrected to show this award.
