THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show the award of the Joint Service Commendation Medal with First Oak Leaf Cluster.
2. The applicant essentially states that she received her orders for the Joint Service Commendation Medal with First Oak Leaf Cluster after she out-processed the United States Army Transition Center at Fort Myer, Virginia.
3. The applicant provides her DD Form 214; separation orders, dated 9 May 2007, which reassigned her to the United States Army Transition Center, Fort Myer, on 22 January 2008, and retired her from the Regular Army on 31 March 2008 and placed her on the retired list on 1 April 2008; and awards orders, a certificate, and a citation which awarded her the Joint Service Commendation Medal with First Oak Leaf Cluster in support of this application.
CONSIDERATION OF EVIDENCE:
1. The applicant's military records show that she enlisted in the Regular Army on 16 March 1988, and that she culminated a 20-year active duty career when she was released from active duty on 31 March 2008 by reason of sufficient time for retirement and placed on the Retired List on 1 April 2008. Her DD Form 214 essentially shows that she served in military occupational specialties 42A (Human Resources Specialist) and 63B (Wheeled Vehicle Mechanic) during her
Army career. This document also essentially shows that she was awarded the Joint Service Commendation Medal, the Army Commendation Medal with Second Oak Leaf Cluster, the Joint Service Achievement Medal with Second Oak Leaf Cluster, the Joint Meritorious Unit Award with Second Oak Leaf Cluster, the Army Good Conduct Medal (6th Award), the National Defense Service Medal with One Bronze Service Star, the Southwest Asia Service Medal with two bronze service stars, the Global War on Terrorism Service Medal, the Korea Defense Service Medal, the Noncommissioned Officer Professional Development Ribbon with Numeral "2," the Army Service Ribbon, the Overseas Service Ribbon (2nd Award), the Kuwait Liberation Medal awarded by the Kingdom of Saudi Arabia, the Kuwait Liberation Medal awarded by the Government of Kuwait, the Driver and Mechanic Badge with Mechanic Badge (2nd Award), the basic Parachutist Badge, the Presidential Service Badge, and the Joint Chiefs of Staff Identification Badge.
2. The applicant's DD Form 214 shows that she was awarded the Joint Service Commendation Medal. However, records show that she was awarded the Joint Service Commendation Medal by Headquarters, United States European Command Permanent Orders Number 288-02, dated 15 October 1999; and the Joint Service Commendation Medal with First Oak Leaf Cluster by Defense Information Systems Agency, Arlington, Virginia Military Personnel Awards Order Number 2008-016, dated 22 January 2008.
3. The Joint Service Commendation Medal was authorized by the Secretary of
Defense on 25 June 1963. It is awarded in the name of the Secretary of Defense to members of the Armed Forces of the United States who, while assigned to a joint activity after 1 January 1963, distinguished themselves by meritorious achievement or service. Army Regulation 600-8-22 (Military Awards) provides, in pertinent part, that an oak leaf cluster is awarded to denote the second and succeeding awards of certain decorations, among which is the Joint Service Commendation Medal.
DISCUSSION AND CONCLUSIONS:
1. The applicant contends that her DD Form 214 should be corrected to show the award of the Joint Service Commendation Medal with First Oak Leaf Cluster.
2. Orders awarded the applicant two awards of the Joint Service Commendation Medal, but only one is listed on her DD Form 214. Therefore, she is entitled to correction of her military records to show the award of the Joint Service Commendation Medal with First Oak Leaf Cluster.

BOARD DETERMINATION/RECOMMENDATION:
1. The Board determined that the evidence presented was sufficient to warrant a recommendation for relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by showing the award of the Joint Service Commendation Medal with First Oak Leaf Cluster.
2. The Board wants to thank the applicant for the sacrifices she made in service to the United States throughout her military career, and especially during Operations DESERT SHIELD and DESERT STORM. The applicant and all Americans should be justifiably proud of her honorable service in arms.
