	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) with the period ending 27 May 2006 to show award of two Army Achievement Medals, the Army Commendation Medal, and the Army Reserve Components Achievement Medal.  He further requests that his DD Form 214 be corrected to show he completed the Primary Leadership Development Course and the Medical Fieldcraft Training Course.

2.  The applicant states, in effect, that when he separated from active duty the above mentioned awards/courses were omitted from his DD Form 214.

3.  The applicant provides his DD Form 214; a Medical Fieldcraft Certificate of Training, dated 21 May 2004; a DA Form 1059 (Service School Academic Evaluation Report), dated 10 March 2005; Headquarters, Dwight David Eisenhower Army Medical Center (DDEAMC) Permanent Order Number 128-3, dated 8 May 2006; Headquarters, 5501st United States Army Hospital (USAH) Permanent Orders 198-24, dated 17 July 2002; Headquarters, 5501st USAH Permanent Orders 254-1, dated 12 September 2004; and a 5501st USAH memorandum, dated 10 January 2004.

CONSIDERATION OF EVIDENCE:

1.  The applicant enlisted in the U.S. Army Reserve on 15 February 2000.  He was ordered to active duty in Support of Operation Enduring Freedom on 
29 November 2004.

2.  The applicant submitted Headquarters, 5501st USAH Permanent Order Number 198-24, dated 17 July 2002, that shows he was awarded the Army Achievement Medal 1st award.

3.  The applicant submitted a memorandum from the 5501st USAH, dated 
10 January 2004, that shows he was awarded the Army Reserve Components Achievement Medal for the period 15 February 2000 through 14 February 2003.

4.  The applicant submitted a Medical Fieldcraft Certificate of Training, dated 
21 May 2004, that shows he completed the 2-week Medical Fieldcraft Training Course on 21 May 2004.

5.  The applicant submitted Headquarters, 5501st USAH Permanent Order Number 254-1, dated 12 September 2004, that shows he was awarded the Army Achievement Medal 2nd award.

6.  The applicant submitted a DA Form 1059, dated 10 March 2005, that shows he completed the 4-week Primary Leadership Development Course on 10 March 2005.

7.  The applicant submitted Headquarters, DDEAMC Permanent Order Number 128-3, dated 8 May 2006, that shows he was awarded the Army Commendation Medal.

8.  On 27 May 2006, the applicant was honorably released from active duty after completing 1 year, 5 months, and 29 days of creditable active service with no time lost.  

9.  His DD Form 214 with the period ending 27 May 2006 shows that he was awarded the National Defense Service Medal, the Global War on Terrorism Service Medal, the Noncommissioned Officer Professional Development Ribbon, the Army Service Ribbon, and the Armed Forces Reserve Medal with "M" Device.  Item 14 (Military Education) shows the entry "NONE//NOTHING FOLLOWS."

10.  Army Regulation 635-5 (Separation Documents) prescribes the separation documents prepared for Soldiers upon retirement, discharge, or release from active military service or control of the Army.  It establishes standardized policy for the preparation of the DD Form 214.  It states for item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized (All Periods of Service)) list awards and decorations for all periods of service in the priority sequence specified in AR 600-8-22.  Each entry will be verified by the SoldierÂ’s records.  Do not use abbreviations.

11.  Army Regulation 635-5 also states for item 14 (Military Education), list in-service training course; title, number of weeks, year successfully completed during this period of service covered by the DD Form 214.  This information is to assist the member after separation in job placement and counseling; therefore, training courses for combat skills will not be listed.

DISCUSSION AND CONCLUSIONS:

1.  Evidence of record shows that the applicant was awarded two Army Achievement Medals, the Army Commendation Medal, and the Army Reserve Components Achievement Medal.  His 27 May 2006 DD Form 214 should be amended to add these awards.


2.  Evidence of record shows that the applicant completed the 2-week Medical Fieldcraft Training Course in 2004 and the 4-week Primary Leadership Development Course in 2005.  Therefore, his DD Form 214 should be corrected to add the Medical Fieldcraft Training Course, 2 weeks (2004) and the Primary Leadership Development Course, 4 weeks (2005) in item 14.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  amending his DD Form 214 for the period ending 27 May 2006 in item 13 to add the Army Achievement Medal 2nd award, the Army Commendation Medal, and the Army Reserve Components Achievement Medal; and 

	b.  amending his DD Form 214 for the period ending 27 May 2006 in item 14 to add the Medical Fieldcraft Training Course, 2 weeks (2004); and the Primary Leadership Development Course, 4 weeks (2005).
