THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release of Discharge from Active Duty) to show: a. all of the awards that he is entitled to receive specifically the Army Commendation Medal (fourth award), Army Achievement Medal (fourth award), Noncommissioned Officer (NCO) Professional Development Ribbon for completion of the Primary Leadership Development Course (PLDC), Combat Infantryman Badge, Expert Infantryman Badge, Parachutist Badge, Honduran Parachutist Badge, and the German Schutzenschnur Marksmanship Badge; b. all of his professional development training; and c. his service in Iraq from 14 February to 18 July 2003. 2. The applicant states that he wants his DD Form 214 corrected to add his first period of service in Iraq from 14 February to 18 July 2003. He also wants his DD Form 214 to show all awards, medals, and decorations not listed. 3. The applicant provides, in support of his application, copies of his DD Form 214, DD Form 2216E (Hearing Conservation Data), orders, certificate and narrative for two Army Commendation Medals and orders for award of the Combat Infantryman Badge. CONSIDERATION OF EVIDENCE: 1. On 16 March 1999, the applicant enlisted in the Regular Army. He completed his initial training and was awarded military occupational specialty (MOS) 96U (Tactical Unmanned Vehicle Operator). 2. Permanent Orders 207-385, United States Army Training Center, Fort Benning, Georgia, dated 26 July 1999, awarded the applicant the Parachutist Badge. 3. During the period from 6 August to 5 September 2002 the applicant attended and successfully completed PLDC at Fort Bragg, North Carolina. 4. Permanent Orders 252-06, 325th Airborne Infantry Regiment, dated 9 September 2002, awarded the applicant the Army Good Conduct Medal (first award). 5. Permanent Orders 127-58, Task Force Falcon, Baghdad, Iraq, dated 7 May 2003, awarded the applicant the Combat Infantryman Badge for engaging the enemy in active ground combat from 24 March to 28 April 2003. 6. Permanent Orders Number 194-01, 325th Airborne Infantry Regiment, dated 15 July 2003, awarded the applicant the Army Commendation Medal for meritorious service as a fire team leader in Iraq from 14 February to 18 July 2003 (5 months and 5 days). 7. Permanent Orders 335-10, 203rd Personnel Services Battalion, dated 1 December 2005, awarded the applicant the Army Good Conduct Medal (second award) for the period of service from 17 March 2002 through 16 March 2005. 8. Permanent Orders Number 105-23, 172nd Stryker Brigade Combat Team, dated 15 April 2006, awarded the applicant the Army Commendation Medal for meritorious service in support of Operation Iraqi Freedom for the period of service from 16 August 2005 to 15 August 2006. 9. On 13 March 2007, the applicant was honorably discharged from the United States Army. He had attained the rank of staff sergeant, pay grade E-6, and had completed 7 years, 11 months, and 28 days of creditable active duty service. 10. Item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) of the applicant's DD Form 214 lists his awards as the Army Commendation Medal, Valorous Unit Award, Army Good Conduct Medal, National Defense Service Medal, Global War on Terrorism Service Medal, Iraq Campaign Medal, Army Service Ribbon, and the Overseas Service Ribbon. It does not show award of four Army Achievement Medals, Noncommissioned Officer Professional Development Ribbon, Combat Infantryman Badge, Honduran Parachutist Badge or the German Schutzenschnur Marksmanship Badge. 11. Item 14 (Military Education) of the applicant's DD Form 214 lists his MOS training but does not show completion of PLDC. 12. Item 18 (Remarks) of the applicant's DD Form 214 states that he served in Iraq from 15 August 2005 to 6 December 2006. It does not show any other service in Iraq. 13. The applicant also requests the issuance of additional medals and badges. Army Retirees and Veterans may submit requests to Headquarters, United States Army Human Resources Command (AHRC–PDO–PA), 1 Reserve Way, St. Louis, Missouri 63132–5200. Requests must include a copy of the retiree’s/veteran’s separation or discharge paperwork and any other supporting documentation to substantiate the request. Based on the information provided there will be no further discussion in this record of proceeding concerning the issuance of additional medals and badges. 14. Army Regulation 600-8-22 (Military Awards) shows that the NCO Professional Development Ribbon was established by the Secretary of the Army on 10 April 1981. It is awarded to members of Active Army, Army National Guard, and U.S. Army Reserve Soldiers for successful completion of designated NCO professional development courses. 15. A directive issued by the Department of Defense on 16 April 2008 designated the following four phases of Operation Iraqi Freedom as qualifying for a campaign star: the Liberation of Iraq, 19 March 2003 to 1 May 2003; Transition of Iraq, 2 May 2003 to 28 June 2004; Iraqi Governance, 29 June 2004 to 15 December 2005; and National Resolution, 16 December 2005 to a date to be decided. A bronze service star will be awarded for wear on the Iraq Campaign Medal for participation in each campaign. 16. Army Regulation 635-5 (Separation Documents) provides detailed instructions for completing separation documents, including the DD Form 214. It provides, in pertinent part, for an entry in block 12f of the total amount of foreign service completed during the current period of active duty. It further provides that Item 18 (Remarks) will contain the following entry for Regular Army Soldiers: "SERVICE IN (NAME OF COUNTRY DEPLOYED) FROM (inclusive dates for example, YYYYMMDD - YYYYMMDD)." DISCUSSION AND CONCLUSIONS: 1. The applicant contends that his DD Form 214 should be corrected to show his first period of service in Iraq from 14 February to 18 July 2003. He also contends that his DD Form 214 should be corrected to show all his awards. He specifically identifies the Army Commendation Medal (four awards), Army Achievement Medal (four awards), a Noncommissioned Officer Professional Development Ribbon, Combat Infantryman Badge, Expert Infantryman Badge, Parachutist Badge, Honduran Parachutist Badge, and the German Schutzenschnur Marksmanship Badge. 2. The evidence shows the applicant served in Iraq from 14 February to 18 July 2003 and from 18 August 2005 to 6 December 2006. Therefore, his DD Form 214 should be corrected to show this overseas service. 3. The evidence shows the applicant served in Iraq during three campaigns. Therefore, he should be awarded three bronze service stars to be worn on his already-awarded Iraq Campaign Medal. 4. Permanent orders show the applicant was awarded the Army Commendation Medal (second award), the Army Good Conduct Medal (second award), the Combat Infantryman Badge, and the Parachutist Badge. Therefore, his records should be corrected to show these awards and the badge. 5. The available evidence also shows the applicant successfully completed PLDC. Therefore, his records should be corrected to show this training and award of the Noncommissioned Officer Professional Development Ribbon. 6. There is no available evidence showing the applicant was authorized more than the second awards of the Army Commendation Medal or the Army Achievement Medal. The available evidence does not show the applicant's marksmanship qualifications or that he is authorized the Expert Infantryman Badge, Honduran Parachutist Badge, and the German marksmanship badge. Therefore, the evidence is insufficient to warrant the requested relief. The Board determined that the evidence presented was sufficient to warrant a recommendation for partial relief. As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by: a. deleting from Item 13 of his DD Form 214 the Army Commendation Medal, Army Good Conduct Medal, and the Iraq Campaign Medal; b. adding to Item 13 of his DD Form 214 the Army Commendation Medal (second award), Army Good Conduct Medal (second award), Iraq Campaign Medal with three bronze service stars, Noncommissioned Officer Professional Development Ribbon, Combat Infantryman Badge and the Parachutist Badge; c. adding to Item 14 of his DD Form 214 the “Primary Leadership Development Course, 4 weeks, September 2002”; and d. adding to Item 18 of the DD Form 214 "Service in Iraq from 20030214 to 20030718." 2. The Board further determined that the evidence presented is insufficient to warrant a portion of the requested relief.
