         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests correction of item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Army Commendation Medal, four awards of the Army Achievement Medal and the Noncommissioned Officers (NCO) Professional Development Ribbon (NPDR) with Numeral 2. He also requests correction of item 14 (Military Education) on his DD Form 214 to show completion of the Basic NCO Course, the Primary Leadership Development Course, the Unit Prevention Leader Certification Course, the Small Group Instructor Training Course, Dragon Certification, the Unit Armorer Course, the Combat Lifesaver Course, Air Assault School and the 3rd U.S. Infantry (The Old Guard) Regimental Orientation Program.

3. The applicant makes no additional statements. In support of his application, he submits five award certificates, one for the Army Commendation Medal and four certificates for the Army Achievement Medal; two DA Forms 1059 (Service School Academic Evaluation Report) for the Basic NCO Course and the Primary Leadership Development Course; and certificates of training for the abovementioned courses.

4. The applicant’s military records show that he enlisted on 5 April 1995, trained as an infantryman and attained the rank of staff sergeant. On 11 October 2000, the applicant was honorably discharged.

5. The applicant’s DD Form 214 does not show the Army Commendation Medal, four awards of the Army Achievement Medal or the NPDR with Numeral 2 as authorized awards. His DD Form 214 does show three awards of the Army Achievement Medal.

6. Item 14 (Military Education) on the applicant’s DD Form 214 shows completion of the Unit Armorer Course in 1996, the Primary Leadership Development Course in 1998 and the Basic NCO Course in 1999. Therefore, these portions of the applicant’s requests will not be addressed further in these proceedings.

7. The applicant provided an award certificate, dated 28 August 1998, which shows he received the Army Commendation Medal for meritorious service during the period 31 October 1995 to 1 October 1998. The certificate also shows this award was published by 2d Brigade, 101st Airborne Division, Permanent Orders #240-04.

8. The applicant provided an award certificate, dated 21 July 1999, which shows he received the Army Achievement Medal for meritorious achievement on
17 April 1999. The certificate also shows this award was published by Headquarters, 3rd Infantry Regiment, Permanent Orders #202-18.

9. The applicant provided an award certificate, dated 30 March 1999, which shows he received the Army Achievement Medal for meritorious achievement during the period 3 March 1999 to 22 March 1999. The certificate also shows this award was published by Headquarters, 3rd Infantry Regiment, Permanent Orders #89-30.

10. The applicant provided an award certificate, dated 1 April 1997, which shows he received the Army Achievement Medal for outstanding performance during the period 8 March 1997 to 19 March 1997. The certificate also shows this award was published by Headquarters, 1st Battalion, 502nd Infantry, Permanent Orders #03-30.

11. The applicant provided an award certificate, dated 5 August 1996, which shows he received the Army Achievement Medal for meritorious service during the Eagle Flight 1 Competition. The certificate also shows this award was published by Headquarters, 1st Battalion, 502nd Infantry, Permanent Orders #01-71.

12. The applicant provided DA Form 1059 (Service School Academic Evaluation Report), dated 23 September 1999, which shows that he attended the Basic NCO Course from 4 August 1999 to 23 September 1999 and graduated with an academic average of 97.76%.

13. The applicant provided DA Form 1059 (Service School Academic Evaluation Report), dated 6 February 1998, which shows that he attended the Primary Leadership Development Course from 7 January 1998 to 6 February 1998 and graduated with an academic average of 93.8%.

14. The applicant provided a certificate of training which shows he completed the 40-hour Unit Prevention Leader Certification Course on 11 February 2000.

15. The applicant provided a certificate of training which shows he completed the 40-hour Small Group Instructor Training Course on 29 January 1999.

16. The applicant provided a certificate of training which shows he completed the 80-hour Dragon Certification on 29 February 1996.

17. The applicant provided a certificate of training which shows he completed the 40-hour Combat Lifesaver Course on 20 June 1997.

18. The applicant provided a certificate of training which shows he completed Air Assault School on 5 September 1996. His DD Form 214 shows the Air Assault Badge as an authorized award.

19. The applicant provided a certificate of training which shows he completed the 3rd U.S. Infantry (The Old Guard) Regimental Orientation Program which was held from 26 October 1998 to 20 November 1998.

20. Army Regulation 600-8-22 (Military Awards) shows that the NonCommissioned Officer Professional Development Ribbon was established by the Secretary of the Army on 10 April 1981. It is awarded to members of Active Army, Army National Guard, and US Army Reserve soldiers for successful completion of designated NCO professional development courses. Numerals
2, 3, and 4 are used to denote the satisfactory completion of the respective levels of NCO professional development courses. Primary level is awarded for completion of Primary Leadership Development Course; basic level (Numeral 2) is awarded for completion of Basic NCO Course; advanced level (Numeral 3) is awarded for advanced NCO courses; and senior level (Numeral 4) is awarded for completion of U.S. Army Sergeants Major Academy.

21. Army Regulation 635-5 establishes the policies and procedures for completion and distribution of the DD Form 214. In pertinent part it states that item 14 (military education) will list formal in service training courses
of 40 hours or more successfully completed during the period of service covered by title, length in weeks, and month and year completed. This information is to assist the soldier after separation in job placement and counseling; therefore, training courses for combat skills are not listed.

CONCLUSIONS:

1. The applicant received the Army Commendation Medal and four awards of the Army Achievement Medal. Therefore, he is entitled to correction of his DD Form 214 to show these awards.

2. The applicant completed the Primary Leadership Development Course and the Basic NCO Course. Therefore, the applicant is entitled to correction of his DD Form 214 to show award of the NPDR with Numeral 2.

3. The applicant completed the 40-hour Unit Prevention Leader Certification Course on 11 February 2000, the 40-hour Small Group Instructor Training Course on 29 January 1999, the 80-hour Dragon Certification on 29 February 1996, the 40-hour Combat Lifesaver Course on 20 June 1997, and the 3rd U.S. Infantry (The Old Guard) Regimental Orientation Program. Therefore, he is entitled to correction of item 14 (Military Education) on his DD Form 214 to show these courses.

4. The applicant completed Air Assault School on 5 September 1996. However, in accordance with the governing regulation, training courses for combat skills are not listed on the DD Form 214.
5. In view of the foregoing findings and conclusions, it would be appropriate to correct the applicant’s records, but only as recommended below.

RECOMMENDATION:

1. That all of the Department of the Army records related to this case be corrected:

         a. by deleting the entry “ARMY ACHIEVEMENT MEDAL (3RD AWARD)” in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on the DD Form 214 of the individual concerned;

         b. by adding the entries “Army Commendation Medal”, “Army Achievement Medal (4th Award)” and “Noncommissioned Officer Professional Development Ribbon with Numeral 2” in item 13 on his DD Form 214; and

         c. by adding the entries “Unit Prevention Leader Certification Course,
1 week, February 2000”, “Small Group Instructor Training Course, 1 week, January 1999”, “Dragon Certification, 2 weeks, February 1996”, “Combat Lifesaver Course, 1 week, June 1997”, and “3rd U.S. Infantry (The Old Guard) Regimental Orientation Program, 4 weeks, November 1998” in item 14 (Military Education) on his DD Form 214.

2. That so much of the application as is in excess of the foregoing be denied.
