	The Board considered the following evidence:

	Exhibit A - Application for correction of military records.

	Exhibit B - Military Personnel Records (including advisory opinion, if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests, in effect, correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show specialist/pay grade E-4, his date of birth to show 14 May 1977, Service Members Group Life Insurance (SGLI) in the amount of $400,000.00, award of the Good Conduct Medal and two Army Achievement Medals.

2.  The applicant states that he was promoted while he was in Kuwait and was awarded the Good Conduct Medal and two Army Achievement Medals that are not shown on his DD Form 214.

3.  The applicant provides a DA Form 4187 (Personnel Action); a copy of his DD Form 214; a State of Mississippi Birth Certificate; a Certificate of Marriage; Bravo Company, 215th Forward Support Battalion Permanent Orders Number 186-004, dated 5 August 2002; a DA Form 638 (Recommendation for Award), dated 15 February  2002; a DA Form 638, dated 27 May 2002; US Army Human Resources Command Orders Number C-12-328244, dated 2 December 2003; and two Defense, Finance and Accounting Service Military Leave and Earning Statements in support of the application. 

CONSIDERATION OF EVIDENCE:

1.  The applicant is currently serving as a member of the United States Army Reserve in the grade of specialist/pay grade E-4.

2.  Item 4a (Grade, Rate, or Rank) of the applicant's DD Form 214 contains the entry "PFC [private first class]."  Item 4b (Pay Grade) of the applicant's DD Form 214 contains the entry "E3."

3.  A DA Form 4187, dated 1 February 2002, shows that the applicant was advanced to the grade of specialist/pay grade E-4 effective 1 February 2002 with a date of rank of 1 February 2002.

4.  Item 5 (Date of Birth) of the applicant's DD Form 214 contains the entry "19770626 [26 June 1977]."

5.  The applicant provided a copy of a State of Mississippi Birth Certificate which shows his date of birth as 14 May 1977.

6.  Item 13 (Decorations, Medal, Badges, Citation and Campaign Ribbons Awarded or Authorized) does not show award of the Good Conduct Medal or Army Achievement Medal with 1st Oak Leaf Cluster.

7.  Bravo Company, 215th Forward Support Battalion Permanent Orders Number 186-004, dated 5 August 2002, show that the applicant was awarded the First Award of the Good Conduct Medal for the period 25 August 1999 through 24 August 2002.

8.  A DA Form 638, dated 15 February 2002, shows that the applicant was recommended and approved for award of the Army Achievement Medal for meritorious achievement while assigned to the 15th Forward Support Battalion as a Track Vehicle Mechanic in support of Operation Enduring Freedom during the period 16 November 2001 through 4 April 2002.  This form further shows that Headquarters, 15th Forward Support Battalion Permanent Orders Number 
126-26 awarded the applicant the Army Achievement Medal on 6 May 2002.

9.  A DA Form 638, dated 27 May 2002, shows that the was applicant was recommended and approved for award of the Army Achievement Medal for meritorious service while assigned to Bravo Company, 215 Forward Support Battalion as a track vehicle repairer during the period 23 March 2000 through 23 August 2002.  This form further shows that the applicant was previously awarded the Army Achievement Medal.  This form also shows that Headquarters, 15th Forward Support Battalion Permanent Orders Number 171-01 awarded the applicant the Army Achievement Medal on 10 July 2002.

10.  Item 10 (SGLI Coverage) of the applicant's DD Form 214 contains the entry "$250,000.00."  

11.  The applicant provided two Defense, Finance and Accounting Service Military Leave and Earning Statements for pay date 24 August 1999 and
15 March 2006, which show that his SGLI coverage was in the amount of $400,000.00.

DISCUSSION AND CONCLUSIONS:

1.  Evidence of records show that the applicant was promoted to specialist/pay grade E-4 prior to his separation from active duty.  Therefore, it would be appropriate in this case to amend his DD Form 214 to show that he was separated in the rank of specialist/pay grade E-4.


2.  Evidence of record shows that the applicant's date of birth is 14 May 1977.  The applicant's DD Form 214 incorrectly shows his date of birth as 26 June 1977. Therefore, the applicant's DD Form 214 should be amended to show 14 May 1977 as his date of birth

3.  Orders show the applicant was awarded the First Award of the Good Conduct Medal for the period 25 August 1999 through 24 August 2002.  Therefore, his records should be amended to show this award.

4.  Orders show the applicant was awarded two Army Commendation Medals.  Therefore, he is entitled to correction of his records to show award of the Army Commendation Medal with 1st Oak Leaf Cluster.

5.  The applicant contends that the SGLI coverage amount shown on his DD Form 214 should be amended to show the amount of $400,000.00.  Pay records indicate that the applicant paid a premium for $400,000.00 of SGLI coverage.  Therefore, it would be appropriate to amend item 10 of his DD Form 214 to show SGLI coverage in the amount of $400,000.00.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by:

	a.  deleting in item 4a  " PFC"  and adding "SPC";

	b.  deleting in item 4b "E3" and adding "E4";

	c.  deleting in item 5 "19770626" and adding "19770514"; 

	
	d.  deleting in item 10 "$250,000.00" and adding "$400,000.00"; and
	
	e.  adding in item 13 "Good Conduct Medal and Army Achievement Medal with 1st Oak Leaf Cluster."
