THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests the amount of Servicemen's Group Life Insurance (SGLI) coverage shown on his DD Form 214 (Certificate of Release or Discharge from Active Duty) with a separation date of 20 June 2013 be changed to show $400,000.00.

2.  The applicant states he updated his SGLI coverage on 28 March 2013 to $400,000.00.

3.  The applicant provides:

* DD Form 214
* Servicemembers' Group Life Insurance Election and Certificate (SGLV 8286), dated 28 March 2013

CONSIDERATION OF EVIDENCE:

1.  Having previously served in the Arizona Army National Guard and on active duty in the U.S. Navy, the applicant enlisted in the U.S. Army Reserve (USAR) on 24 September 2002.

2.  On 3 August 2003, he entered active duty.  He immediately reenlisted on 
7 December 2004, 6 October 2005, and 17 December 2010.  He remained on active duty until his retirement.

3.  An SGLV 8286, dated 7 November 2008, states he was automatically, by law, insured for $400,000.00.  If he desired less coverage or to decline coverage he had to indicate such election on the form.  He did not elect less coverage or decline coverage.

4.  On an SGLV 8286, dated 22 March 2011, he specifically elected SGLI coverage in the amount of $400,000.00.

5.  On an SGLV 8286, dated 28 March 2013, he specifically elected SGLI coverage in the amount of $400,000.00.

6.  On 20 June 2013, he was retired and placed on the Retired List by reason of permanent disability.  Item 10 (SGLI Coverage Amount) of his DD Form 214 shows the amount $250,000.00.

7.    Army Regulation 635-5 (Separation Documents), then in effect, established the standardized policy for preparing and distributing the DD Form 214.  The regulation directed that the purpose of the separation document was to provide the individual with documentary evidence of his or her military service at the time of release from active duty, retirement, or discharge.  It was important that information entered on the form be complete and accurate and reflected the conditions as they existed at the time of separation.

DISCUSSION AND CONCLUSIONS:

The SGLV 8286's dated 7 November 2008, 22 March 2011, and 28 March 2013, clearly show the applicant consistently elected SGLI coverage in the amount of $400,000.00 while he was on active duty.  Therefore, the entry in 
item 10 of his DD Form 214 should be corrected to show $400,000.00.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant a recommendation for relief.  As a result, the Board recommends that all Department of the Army records of the individual concerned be corrected by changing the amount of SGLI coverage in item 10 of his DD Form 214 for the period ending 20 June 2013 to $400,000.00.
