THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests, in effect, that item 12a (date entered AD
[active duty] this period) on his May 2004 Department of Defense Form 214
(Certificate of Release or Discharge from Active Duty) be corrected.
2. The applicant states the date should read 16 January 2003 vice 23
August 1999.
3. The applicant provides a copy of orders bringing him on active duty
effective 16 January 2003 and a copy of his retirement point summary
statement.
CONSIDERATION OF EVIDENCE:
1. The applicant's existing Official Military Personnel File indicates the
applicant has been a member of various components of the United States Army
since 1979.
2. His Army National Guard Retirement Points History Statement, dated
7 November 2003 and provided by the applicant in support of his request,
noted that in June 2001 he returned to duty as a member of the Army
National Guard following a break in his military service. He was briefly
transferred to the United States Army Reserve Control Group (Reinforcement)
between December 2002 and January 2003 but returned to his Army National
Guard unit on 13 January 2003.
3. Orders published by the State of Colorado Department of Military
Affairs ordered the applicant to active duty in support of Operation
Enduring Freedom on 16 January 2003. He remained in an active status until
8 May 2004 when he was discharged from active duty and his name placed on
the Temporary Disability Retired List.
4. The separation document, executed as part of his May 2004 discharge
action indicates in item 12a that he entered active duty on 23 August 1999
and item 12c (net active service this period) notes he had served on active
duty for a period of 4 years, 8 months, and 16 days, the period which would
have elapsed between 23 August 1999 and 8 May 2004. The applicant actually
served on active duty for a period of 1 year, 3 months, and 23 days between
16 January 2003 and 8 May 2004.
5. The applicant's Retirement Points History Statement indicates that
between 1995 and 2000 the applicant had no military status.
6. His 2004 separation document was prepared by the transition center at
Fort Carson, Colorado.
DISCUSSION AND CONCLUSIONS:
1. The evidence provided by the applicant supports his contention that he
was ordered to active duty on 16 January 2003 and not 23 August 1999 as his
May 2004 separation document suggests.
2. Item 12a on the applicant's 2004 separation document appears to have
been entered in error which resulted in the incorrect service computation
entered in item 12c. In view of the foregoing, it would be appropriate to
correct item 12a on the applicant's May 2004 separation document to show
that he entered active duty on 16 January 2003 and that item 12c be
corrected to show that his period of active duty between January 2003 and
May 2004 encompassed 1 year, 3 months, and 23 days.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected;
a. by showing in item 12a on his May 2004 separation document that he
entered active duty on 16 January 2003; and
b. by showing in item 12c that his period of active service
encompassed
1 year, 3 months, and 23 days.
