The Board considered the following evidence:

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant requests that his DD Form 214 (Certificate of Release or
Discharge from Active Duty) for the period ending 7 October 2003 be amended
to add the Meritorious Service Medal (MSM), a second award of the Military
Outstanding Volunteer Service Medal (MOVSM), a third award of the National
Defense Service Medal (NDSM), and the Global War on Terrorism Service Medal
(GWOTSM).
2. The applicant makes no additional statement.
3. The applicant provides release from active duty orders dated 5
September 2003; his DD Form 214; an MSM award certificate; his
recommendation for award of the MSM; a Certificate of Achievement dated 9
September 2003; and his recommendation for the second award of the MOVSM.
CONSIDERATION OF EVIDENCE:
1. The applicant was inducted into the Army on 17 September 1969. He
enlisted in the Regular Army on 24 September 1969. He was honorably
released from active duty on 29 March 1972. He enlisted in the U. S. Army
Reserve (USAR) in 1978. He remained with a Troop Program Unit in the USAR
until 3 December 1994, when he reenlisted for the Individual Ready
Reserve/Individual Mobilization Augmentee. He was ordered to active duty
on 8 October 2001 in support of Operation Noble Eagle and assigned to the
U. S. Army Special Operations Command, Fort Bragg, NC.
2. On 18 July 2003, the applicant was recommended for a second award of
the MOVSM. The recommendation was approved by Brigadier General B___ and
the certificate was prepared on 9 September 2003.
3. The applicant was awarded the MSM on Headquarters, U. S. Army Special
Operations Command Permanent Order Number 252-7 dated 9 September 2003.
4. The applicant was released from active duty on 7 October 2003. His DD
Form 214 does not list the MSM or the GWOTSM. It lists one award of the
MOVSM and two awards of the NDSM.
5. The applicant was assigned to the Retired Reserve on 8 February 2004.
6. Army Regulation 600-8-22 (Military Awards) provides that the MOVSM may
be awarded to members of the Armed Forces of the United States and their
Reserve Components who, subsequent to 31 December 1992, perform outstanding
volunteer community service of a sustained, direct and consequential
nature. To qualify for award of the MOVSM a service member's volunteer
service must meet the following requirements: (1) be to the civilian
community, to include the military family community; (2) be significant in
nature and produce tangible results; (3) reflect favorably on the Military
Service and the Department of Defense; and (4) be of a sustained and direct
nature.
7. Military Personnel Message Number 02-150, subject: Reinstatement of
the National Defense Service Medal, referenced Office of the Deputy
Secretary of Defense Memorandum, same subject, dated 26 April 2002. The
memorandum authorized reinstatement of the NDSM. It stated that the NDSM
is awarded for honorable active service and includes the following
inclusive periods: 27 June 1950 to 27 July 1954; 1 January 1961 to 14
August 1974; 2 August 1990 to 30 November 1995; and 11 September 2001
to a date to be determined. It stated that, as a one-time only exception,
members of the Army National Guard and U. S Army Reserve who were part of
the Selected Reserve in good standing were authorized the NDSM per
Executive Orders 12778 dated 18 October 1991 for the period 2 August 1990
to 30 November 1995. A bronze service star will be worn to signify receipt
of a second or subsequent award.
8. U. S. Army Human Resources Command Message date time group 17 March
2004 disseminated implementing instructions for award of the GWOTSM. This
award is designated for Soldiers who have participated in or served in
support of Global War on Terrorism Operations outside of the designated
area of eligibility determined for award of the Global War on Terrorism
Expeditionary Medal on or after 11 September 2001 to a date to be
determined. All Soldiers on active duty, including Reserve Component
Soldiers mobilized or National Guard Soldiers activated, on or after 11
September 2001 to a date to be determined having served 30 consecutive days
or 60 nonconsecutive days are authorized this award.
DISCUSSION AND CONCLUSIONS:
1. The applicant was awarded the MSM and a second award of the MOVSM on 9
September 2003, just prior to his release from active duty in October 2003.
These awards should be added to his DD Form 214 for the period ending
7 October 2003.
2. The applicant met the eligibility criteria for award of the GWOTSM and
for a third award of the NDSM. These awards should be added to his DD Form
214 for the period ending 7 October 2003.

BOARD DETERMINATION/RECOMMENDATION:
The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief. As a result, the Board recommends that the DD
Form 214 for the period ending 7 October 2003 of the individual concerned
be corrected by adding the Meritorious Service Medal, a second award of the
Military Outstanding Volunteer Service Medal, a third award of the National
Defense Service Medal, and the Global War on Terrorism Service Medal.
