      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that the order showing that he was awarded the
    5th award of the Good Conduct Medal (GCM) be removed from his Official
Military Personnel File (OMPF).

2.  The applicant states that he received the 5th award of the Good Conduct
Medal for the period 3 February 1990 to 2 February 1993; however, he
entered on active duty in September 1990 and received the first award of
the GCM in October 1993.  Somehow his personnel information ended up on a
document awarding the 5th award of that medal intended for a different
Soldier.

3.  The applicant provides a copy of the order awarding him the 5th award
of the GCM, and orders awarding him the first, second, and third awards of
the GCM.

CONSIDERATION OF EVIDENCE:

1.  The applicant was born on 4 February 1964.  The applicant enlisted in
the Army Reserve Delayed Entry Program (DEP) for 8 years on 31 August 1990.
 He was discharged from the DEP upon his enlistment in the Regular Army for
         2 years on 13 September 1990.  He was 26 years old.  The applicant
was awarded the first award of the Good Conduct Medal for the period 13
September 1990 to 12 September 1993, and the second and third awards of the
GCM, as indicated by the orders he enclosed with his application.  The
order awarding him the 5th award of the GCM is also included in his OMPF.
He continued his service as an enlisted Soldier attaining the rank of staff
sergeant, until his appointment as a second lieutenant on 14 September
2001.  He was promoted to first lieutenant in March 2003 and is currently
serving on active duty in Germany.

2.  The Army Good Conduct Medal is awarded to individuals who distinguish
themselves by their conduct, efficiency and fidelity during a qualifying
period of active duty enlisted service.  This period is 3 years except in
those cases when the period for the first award ends with the termination
of a period of Federal military service.

DISCUSSION AND CONCLUSIONS:

1.  The evidence shows that he entered on active duty as indicated above,
and was awarded the first award of the GCM for the period 13 September 1990
to    12 September 1993.  The order awarding him the 5th award is obviously
erroneous.
2.  The applicant's contention is accepted.  The order awarding him the 5th
award of the Good Conduct Medal should be expunged from his record.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
expunging Order Number 33-1627, Headquarters, Fort Carson and Headquarters,
4th Infantry Division (Mechanized), dated 2 February 1993, showing award of
the Good Conduct Medal 5th award.
