      The applicant and counsel if any, did not appear before the Board.

      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that her DD Form 214 (Certificate of Release or
Discharge from Active Duty) be corrected to show that she was not provided
a complete dental exam and all appropriate dental services and treatment
within  90 days prior to her separation.

2.  The applicant states that item 17 of her DD Form 214 was incorrectly
marked, "Yes."  She received a dental examination; however, further
treatment was identified and needed.

3.  The applicant provides a copy of her DD Form 214 and a copy of a dental
record (Standard Form 603A).

CONSIDERATION OF EVIDENCE:

1.  The applicant's DD Form 214 shows that she was discharged because of a
hardship on 16 October 2003.  She had 7 months and 2 days of service.  Item
    17 of her DD Form 214 indicates that she was provided a dental
examination and appropriate dental services and treatment within 90 days of
her separation.

2.  Standard Form 630A shows that, while she did receive a dental
examination on 7 October 2003, the examining dentist indicated that she had
not been provided a complete dental examination and all appropriate dental
services and treatment indicated by the examination within 90 days of
separation.

3.  Army Regulation 635-5 establishes the policies and provisions for the
completion and distribution of the DD Form 214.  It notes that item 17 is
applicable to soldiers who have completed at least 180 days of continuous
active service at the time of separation.  If the soldier had been provided
a complete dental examination, and all appropriate dental services and
treatment had been completed within 90 days prior to separation, the item
is marked “yes.”  The information is obtained by reviewing a soldier’s SF
603 (Health Record-Dental) for validity of exam and all appropriate
treatment.  The information in item 17 is intended to assist the DVA
(Department of Veterans Affairs) in determining whether a veteran is
entitled to continued dental services/care by that agency for dental
defects that had not been completely corrected prior to the individual’s
separation from active duty.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention is accepted.  Evidence shows that she was
not provided the necessary dental treatment and services within 90 days of
her discharge.

2.  Consequently, her 16 October 2003 should be corrected to show that she
was not provided a complete dental exam and all appropriate dental services
and treatment within 90 days prior to separation.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
showing that she was not provided a complete dental exam and all
appropriate dental services and treatment within 90 days prior to
separation.
