      The Board considered the following evidence:

      Exhibit A - Application for correction of military records.

      Exhibit B - Military Personnel Records (including advisory opinion,
if any).

THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant requests that item 17 (Dental Care) on his June 2004
Department of Defense Form 214 (Certificate of Release or Discharge from
Active Duty) be corrected to show that the “no” block was marked, vice the
“yes” block.

2.  The applicant states that his separation document was incorrect and
that he had not finished his dental work.

3.  The applicant provides a copy of his separation document and an extract
from his service dental records.

CONSIDERATION OF EVIDENCE:

1.  Records available to the Board indicate that the applicant served on
active duty between 5 June 2000 and 4 June 2004.  He was released from
active duty, with an honorable characterization of service, at the
completion of his enlistment contract.

2.  The extract from his service dental record indicates that a dentist saw
the applicant on 7 April 2004 and again on 10 May 2004.  The May 2004 entry
notes that the applicant informed the dentist that he would be separating
from the Army and the dentist indicated that there was insufficient time to
initiate and complete treatment “due to short time line.”  The entry
contains a recommendation of the type of dental treatment that was needed.

3.  Item 17 on the applicant’s separation document is marked “yes”
indicating that the applicant “was provided a complete dental exam and all
appropriate dental service and treatment within 90 days prior to
separation.”

4.  Army Regulation 635-5 establishes the policies and provisions for the
completion and distribution of the DD Form 214.  It notes that item 17 is
applicable to Soldiers who have completed at least 180 days of continuous
active service at the time of separation.  If the Soldier had been provided
a complete dental examination, and all appropriate dental services and
treatment had been completed within 90 days prior to separation, the item
is marked “yes.”  The information is obtained by reviewing a Soldier’s SF
603 (Health Record-Dental) for validity of exam and all appropriate
treatment.  The information in item 17 is intended to assist the Department
of Veterans Affairs in determining whether a veteran is entitled to
continued dental services/care by that agency for dental defects that had
not been completely corrected prior to the individual’s separation from
active duty.

DISCUSSION AND CONCLUSIONS:

1.  The applicant's contention is accepted.  Evidence shows that he was not
provided all of the necessary dental treatment and services within 90 days
of his discharge.
 
2.  Consequently, his 31 August 2003 separation document should be
corrected to show that he was not provided a complete dental exam and all
appropriate dental services and treatment within 90 days prior to
separation.

BOARD DETERMINATION/RECOMMENDATION:

The Board determined that the evidence presented was sufficient to warrant
a recommendation for relief.  As a result, the Board recommends that all
Department of the Army records of the individual concerned be corrected by
changing item 17 to show that he was not provided a complete dental exam
and all appropriate dental services and treatment within 90 days prior to
separation.
