         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

         Exhibit A - Application for correction of military
records
         Exhibit B - Military Personnel Records (including
         advisory opinion, if any)

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that her Army Commendation Medal (ARCOM) with 1st oak leaf cluster and the Army Service Ribbon be added to her DD Form 214 (Certificate of Release or Discharge from Active Duty).

3. The applicant states, in effect, that these awards are missing from her listed awards. She submits a copy of her orders for the ARCOM with 1st oak leaf cluster in support of her application.

4. The applicant’s military records show that she served on active duty from 3 November 1994 through 15 September 2002 as an aviation operations specialist. Her records show that she completed her initial entry training in 1995.

5. Her DD Form 214, dated 15 September 2002, does not list the ARCOM with 1st oak leaf cluster or the Army Service Ribbon as authorized awards.

6. The applicant provided a copy of her ARCOM with 1st oak leaf cluster award orders, permanent order number 189-1, dated 8 July 2002.

7. Army Regulation 600-8-22 provides that the Army Service Ribbon is awarded to soldiers effective 1 August 1981 for successful completion of initial entry training.

CONCLUSIONS:

1. The applicant was entitled to award of the Army Service Ribbon upon completion of her initial entry training in 1995 and it should be added to her records.

2. The applicant was awarded the ARCOM with 1st oak leaf cluster on 8 July 2002 and it should be added to her records.

3. In view of the foregoing, the applicant’s records should be corrected as recommended below.












RECOMMENDATION:

That all of the Department of the Army records related to this case for the individual concerned be corrected by adding the ARCOM with 1st oak leaf cluster and the Army Service Ribbon to her DD Form 214, dated 15 September 2002.
