2. The applicant requests correction of his DD Form 214 (Certificate of Release or Discharge from Active Duty) to show award of the Humanitarian Service Medal for participation in support of Operation Uphold Democracy.
3. The applicant states that he served in Haiti during Operation Uphold Democracy. On 10 December 2001, he discovered on the U.S. Total Army Personnel Command (PERSCOM) internet website that the Humanitarian Service Medal was awarded to participants. He believes his service record should be amended to add this award. In support of his application, he submits a supplemental letter; a copy of his DD Form 214; a portion of the Humanitarian Service Medal Operations web page; a Certificate of Achievement; an Armed Forces Expeditionary Medal by-name listing; and award certificate for the Army Commendation Medal.
4. The applicant’s military records show that he served as an enlisted soldier in the U.S. Army Reserve and the Regular Army from 14 March 1989 to 27 January 1993. He was ordered to active duty on 28 January 1993 in the rank of second lieutenant. The applicant was honorably released from active duty on 24 June 1996.
5. The applicant's DD Form 214 does not show entitlement to the Humanitarian Service Medal.
6. The applicant provided a copy of a Certificate of Achievement, dated 16 June 1995, which shows he received recognition for participation in support of the Joint Task Force 180 and Operation Uphold Democracy in Haiti.
7. The applicant also provided a copy of an award certificate, dated 20 April 1995, which shows he was awarded the Army Commendation Medal for meritorious service from 10 September 1994 to 10 November 1994 with the 546th Transportation Company in support of Operation Restore/Uphold Democracy.
8. Army Regulation 600-8-22 provides, in pertinent part, that the Humanitarian Service Medal is awarded to members who distinguished themselves by meritorious direct participation in a Department of Defense approved significant military act or operation of a humanitarian nature. A service member must be on active duty at the time of direct participation, must have directly participated in the humanitarian act or operation within the designated geographical area of operation and within specified time limits, and must provide evidence that substantiates direct participation.
9. The U.S. Total Army Personnel Command has placed the Department of Defense approved Humanitarian Service Medal operations on the Internet. This listing shows that the Joint Task Force 180, Joint Task Force 190, and the Multinational Force Operation Uphold and Restore Democracy were authorized the Humanitarian Service Medal for the period 10 September 1994 to 31 March 1995 for participation in Haiti.
CONCLUSIONS:
1. There is no evidence available to the Board which authorizes award of the Humanitarian Service Medal to the applicant. However, the applicant has provided a Certificate of Achievement authenticated by a general officer and shows he participated in support of the Joint Task Force 180 and Operation Uphold Democracy in Haiti. He also provided an award certificate which shows he was awarded the Army Commendation Medal for meritorious service from 10 September 1994 to 10 November 1994 in support of Operation Uphold Democracy.
2. Upon review of the evidence submitted by the applicant in support of his claim for award of the Humanitarian Service Medal, the Board has determined that this evidence is sufficient as a basis for award of the Humanitarian Service Medal.
3. Therefore, the applicant's DD Form 214 will be amended to show award of the Humanitarian Service Medal.
4. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by showing the individual concerned was awarded the Humanitarian Service Medal.
