2. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her separation date as 25 August 2001.
3. The applicant states that her DD Form 214 incorrectly shows her separation date as 25 November 2001 which is causing her problems in school and employment.
4. The applicant provides a copy of her DD Form 214 and discharge orders in support of her application.
5. The applicant’s military records show that she entered active duty on 15 July 1999. She completed basic and advanced individual training at Fort Jackson, South Carolina. She served in Germany from 2 February 2000 through 24 August 2001 as a light wheel vehicle repairer.
6. Item 12b (Separation Date This Period) of the applicant’s DD Form 214 shows her separation date as 25 November 2001.
7. The applicant provided a copy of Headquarters, U.S. Army, Europe Orders Number 204-001, dated 23 July 2001, which shows her effective date of separation as 25 August 2001.
8. The applicant’s records contain a memorandum, dated 9 August 2001, from the Schweinfurt Transition Center in Germany and addressed to the Commander of the U.S. Army Reserve Command, ATTN: ARPC-PSR-D, St. Louis, Missouri. This memorandum requested that item 12b of the applicant’s DD Form 214 be corrected to show the entry “2001 Year(s) 08 Month(s) 25 day (s).”
9. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. Paragraph 2-6 covers alterations and corrections of the DD Form 214. It states, in pertinent part, that after a soldier departs the Transition Center and/or if distribution of the DD Form 214 has been made, corrections will be made by forwarding a memorandum to the appropriate location requesting a new DD Form 214 or DD Form 215. It further states that corrections of the DD Form 214 of those soldiers who have been discharged or retired without Reserve status will be made by forwarding a memorandum to the Director, National Personnel Records Center, ATTN: Chief, Army Reference Branch, in St. Louis, Missouri.
10. Army Regulation 635-5 also states, in pertinent part, that item 12b (Separation Date This Period) will be completed to show the separation date from active duty.
CONCLUSIONS:
1. The applicant’s DD Form 214 shows her separation date as 25 November 2001.
2. Orders submitted by the applicant in support of her request show she was separated from active duty on 25 August 2001.
3. A memorandum from the Transition Center in Schweifurt Germany was forwarded to the U.S. Army Reserve Personnel Command requesting correction of the applicant’s DD Form 214 to show her separation date as 25 August 2001. However, there is no evidence available to the Board which shows this correction was ever made.
4. It is apparent that there is an administrative error in this case regarding the applicant’s date of separation. Therefore, it would be appropriate to change her DD Form 214 to show her date of separation as 25 August 2001.
5. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected by amending item 12b (Separation Date This Period) of the DD Form 214 of the individual concerned to show her separation date 25 August 2001.
