Military Personnel Records and advisory opinions (if any). THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE: 1. The applicant requests that her DD Form 214 (Certificate of Release or Discharge from Active Duty) be corrected to show in block 12b that she was released from active duty (REFRAD) on 8 September 2009 instead of 29 July 2009 as currently reflected on that form. 2. The applicant states that her DD Form 214 incorrectly reflects that she was REFRAD on 29 July 2009, when in fact she was in a medical hold status until 8 September 2009. 3. The applicant provides a copy of her DD Form 214, a memorandum from the Chief of the Transition Center at Walter Reed Army Medical Center (WRAMC), a memorandum from her commander verifying her service, and a copy of her orders to active duty. CONSIDERATION OF EVIDENCE: 1. The applicant enlisted in the United States Army Reserve (USAR) in the pay grade of E-4 on 24 October 2007 for a period of 8 years, training as a health care specialist, a Non-Prior Service enlistment bonus ($20,000), the Student Loan Repayment Program ($20,000) and the Non-Prior Service Montgomery GI Bill Kicker. 2. She was ordered to active duty for training on 13 November 2007. She completed her basic training at Fort Leonard Wood, Missouri and her advanced individual training (a 52-week course) at Fort Sam Houston, Texas and WRAMC. 3. On 29 July 2009, A DD Form 214 was issued at WRAMC Transition Center that released her from active duty training. However, on 31 July 2009, her commander authored a memorandum which verified her service and indicated that she would be in a medical hold status for the next 43 days pending the delivery and care of her child with an expected permanent change of station (PCS) date of 8 September 2009. 4. Although dated 25 June 2009, the Chief of the WRAMC Transition Center dispatched a memorandum to the Human Resources Command – St Louis (HRC-STL) requesting that the applicant’s DD Form 214 be corrected to reflect a separation date of 8 September 2009 instead of 29 July 2009, as currently reflected. There is no evidence in the available records to show that the change was ever accomplished. DISCUSSION AND CONCLUSIONS: 1. The applicant’s contention that her DD Form 214 should be corrected to reflect that she was REFRAD on 8 September 2009 instead of 29 July 2009 has been noted and found to have merit. 2. The evidence submitted by the applicant clearly shows that she was REFRAD on 8 September 2009 and therefore is entitled to have her records corrected to reflect such. 3. Additionally, the applicant’s DD Form 214 needs to have her service in block 12c (Net Active Service this Period) adjusted accordingly.
