         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

         Exhibit A - Application for correction of military
records
         Exhibit B - Military Personnel Records (including
         advisory opinion, if any)

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that the social security number on his DD Form 214, Report of Transfer or Discharge, be corrected.

3. The applicant states, in effect, that the social security number shown on the DD Form 214 is incorrect and should be corrected to reflect the same number as shown on his application to this Board.

4. In support of his request, he has furnished a copy of what appears to be a printout from the Social Security Administration showing that his social security number is the same as that shown on his induction order.

5. The applicant’s military records show that he was inducted on 4 October 1967 and on 4 September 1969 was placed on the permanent disability retired list.

6. His DD Form 214, Item 3, Social Security Number, reflects a number completely different from that shown on his induction order.

7. Several documents in the applicant’s Military Personnel Records Jacket show his social security number as the same number shown on his DD Form 214; however, even more documents reflect his social security number as the same as that under which he was inducted. The social security number under which he was inducted is also reflected on DD Form 1584 (National Agency Check Request), and is listed on all of his Leave and Earning Statements.

CONCLUSIONS:

1. Evidence provided by the applicant and evidence in his record shows that an error was made and the incorrect social security number was placed on his DD Form 214. Accordingly, it would be appropriate to correct the social security number on his DD Form 214 to reflect the same number as shown on his application to this Board.

2. In view of the foregoing, the applicant’s records should be corrected as recommended below.











RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected by showing that the social security number of the individual concerned reflects the same number as shown on his application to the Army Board for Correction of Military Records.
