2. The applicant requests correction of her DD Form 214 (Certificate of Release or Discharge from Active Duty) to show her correct Social Security Account Number (SSAN) and four awards of the Army Achievement Medal (AAM).
3. The applicant states, in effect, that her SSAN is incorrect and her discharge document only shows three awards of the AAM. In support of her application, she submits a copy of her DD Form 214, her marriage license, her Armed Forces Identification Card, her driver’s license, her SSAN card and four award certificates for the AAM.
4. The applicant’s military records show that she entered active duty on 24 June 1996, served as a personnel management specialist and was honorably discharged on 7 June 2001 for parenthood.
5. Item 3 (Social Security Number) on the applicant’s DD Form 214 shows the entries, “44” as the fourth and fifth digits of her SSAN. Her discharge document shows entitlement to three awards of the AAM.
6. The applicant’s service personnel records show the entries, “66” as the fourth and fifth digits of her SSAN.
7. Records show that a memorandum, dated 3 July 2001, requested that the fourth and fifth digits of the applicant’s SSAN on her DD Form 214 be corrected. The Board notes that the seventh digit of the applicant’s SSAN shown on this memorandum is incorrectly shown as a “3” instead of a “2”.
8. The applicant submitted an award certificate which shows that Permanent Orders Number 166-001, 16 June 1998, published an award of the AAM for the period 1 November 1996 through 22 July 1998.
9. The applicant submitted an award certificate which shows that Permanent Orders Number 325, dated 20 November 2000, published an award of the AAM for the period 15 August 2000 to 15 October 2000.
10. The applicant submitted an award certificate which shows that Permanent Orders Number 076-01, dated 16 March 2001, published an award of the AAM for the period 5 October 1999 to 12 January 2001.
11. The applicant submitted an award certificate which shows that Permanent Orders Number 152-01, dated 1 June 2001, published an award of the AAM for the period 17 September 1998 to 6 June 2001.
CONCLUSIONS:
1. The applicant’s SSAN was incorrectly entered on her DD Form 214, in that, the fourth and fifth digits were incorrectly entered as “44” instead of “66”. Therefore, her DD Form 214 should be corrected at this time.
2. The applicant’s DD Form 214 shows entitlement to three awards of the AAM. Evidence of record shows that the applicant received four awards of the AAM. Therefore, the applicant is entitled to correction of her DD Form 214 to show four awards of the AAM.
3. In view of the foregoing, the applicant’s records should be corrected as recommended below.
RECOMMENDATION:
That all of the Department of the Army records related to this case be corrected:
a. by deleting the fourth and fifth digit entries, “44” in item 3 (Social Security Number) on the DD Form 214 of the individual concerned;
b. by adding the fourth and fifth digit entries, “66” in item 3;
c. by deleting the entry, “ARMY ACHIEVEMENT MEDAL (3RD AWARD) in item 13 (Decorations, Medals, Badges, Citations and Campaign Ribbons Awarded or Authorized) on her DD Form 214; and
d. by showing the entry, “ARMY ACHIEVEMENT MEDAL (4TH AWARD) in item 13 on her DD Form 214.
