2. The applicant requests that his separation document (DD Form 214) be corrected to reflect his correct Social Security Account Number (SSAN).

3. The applicant states, in effect, that the eighth digit of his SSAN is incorrectly listed on his DD Form 214 as a “9,” and that it should be a “4.” He provides a copy of his social security card and DD Form 214 to substantiate his claim.

4. The applicant’s military records were destroyed in the National Personnel Records Center fire of 1973. However, his DD Form 214 shows that he served on active duty for 1 year, 11 months, and 28 days between 24 February 1955 and 21 February 1957, when he was honorably discharged as a private first class and transferred to the USAR.

5. The remarks section of his DD Form 214 contains his SSAN. The eighth digit is a “9.”

6. The applicant’s social security card indicates that the eighth digit of his SSAN is a “4.”

CONCLUSIONS:

1. The applicant’s SSAN was incorrectly entered on his DD Form 214, in that, the eighth digit was incorrectly entered as a “9” instead of a “4.”

2. The applicant’s social security card confirms that the eighth digit of his SSAN should be a “4.”

3. In view of the foregoing, it would be appropriate to correct the applicant’s records as recommended below.

RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected by showing that the eighth digit of SSAN of the individual concerned is a “4.”

BOARD VOTE:

                       GRANT AS STATED IN RECOMMENDATION

                       GRANT FORMAL HEARING

                       DENY APPLICATION
