         The applicant and counsel if any, did not appear before the Board.

         The Board considered the following evidence:

         Exhibit A - Application for correction of military
records
         Exhibit B - Military Personnel Records (including
         advisory opinion, if any)

FINDINGS :

1. The applicant has exhausted or the Board has waived the requirement for exhaustion of all administrative remedies afforded by existing law or regulations.


2. The applicant requests that the eighth and ninth digits of his Social Security Account Number (SSAN) shown in item 3 (Social Security Number) on his DD Form 214 (Report of Transfer or Discharge) be corrected to show, “14” instead of “41.”

3. The applicant states, in effect, that his Social Security Number is incorrect on his DD Form 214. In support of his application, he submits a copy of his SSAN card.

4. The applicant’s military records show that he enlisted on 28 October 1966 for a period of 3 years. He served as a military policeman in Vietnam from
30 November 1967 through 23 November 1968 and was honorably released from active duty on 25 August 1969.

5. Item 3 (Social Security Number) on the applicant’s DD Form 214 shows the eighth and ninth digits as “41.”

6. The applicant’s DD Form 398 (Statement of Personal History), dated
26 October 1966, shows the eighth and ninth digits of his Social Security Number as “14.”

7. In support of his claim, the applicant provided a copy of his SSAN card which shows the eighth and ninth digits of his SSAN as “14.”

CONCLUSIONS:

1. The applicant’s SSAN was incorrectly entered on his DD Form 214, in that, the eighth and ninth digits were incorrectly entered as, “41” instead of “14.” Therefore, his DD Form 214 should be corrected at this time.

2. In view of the foregoing, the applicant’s records should be corrected as recommended below.

RECOMMENDATION:

That all of the Department of the Army records related to this case be corrected:

         a. by deleting the eighth and ninth digit entries, “41” in item 3 (Social Security Number) on the DD Form 214 of the individual concerned;






         b. by adding the eighth and ninth digit entries, “14” in item 3 on his DD Form 214.
