THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:
1. The applicant, the widow of a deceased former service member (FSM), requests correction of item 3 (Social Security Number (SSN)) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) by amending the eighth digit to reflect 3 instead of 2.
2. She states the FSMs SSN was incorrectly entered on his DD Form 214.
3. She provides:
* A training certificate
* His DD Form 214
* His social security account number (SSAN) card
* A death certificate
CONSIDERATION OF EVIDENCE:
1. His records show he enlisted in the Regular Army on 14 April 1983. His
DD Form 4 (Enlistment/Reenlistment Document Armed Forces of the United States) shows his SSN as xxx-xx-xx3x.
2. His record also includes a DD Form 93 (Record of Emergency Data) and permanent orders which show his SSN as xxx-xx-xx3x.
3. The applicant provided a copy of the FSMs course completion certificate for basic combat training. This document lists his SSN as xxx-xx-xx3x.
4. He was honorably released from active duty on 13 April 1987 and transferred to the U.S. Army Reserve Control Group (Reinforcement) for completion of his Reserve service obligation. Item 3 of the DD Form 214 shows his SSN as xxx-xx-xx2x.
5. She provided a copy of the FSMs SSAN card and death certificate. Both documents show his SSN as xxx-xx-xx3x.
6. Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214. The regulation states the purpose of the separation document is to provide the individual documentary evidence of his or her military service. It is important that information entered on the form be complete and accurate.
DISCUSSION AND CONCLUSIONS:
1. The military personnel file shows the FSMs SSN is xxx-xx-xx3x, and this is the SSN that was consistently used throughout his military career.
2. Therefore, his DD Form 214 should be corrected to show the SSN as it is listed on his DD Form 93.
