THE BOARD CONSIDERED THE FOLLOWING EVIDENCE:

1.  Application for correction of military records (with supporting documents provided, if any).

2.  Military Personnel Records and advisory opinions (if any).


THE APPLICANT'S REQUEST, STATEMENT, AND EVIDENCE:

1.  The applicant, the widow of a deceased former service member (FSM), requests correction of item 3 (Social Security Number (SSN)) of his DD Form 214 (Certificate of Release or Discharge from Active Duty) by amending the eighth digit to reflect “3” instead of “2.” 

2.  She states the FSM’s SSN was incorrectly entered on his DD Form 214.

3.  She provides:

* A training certificate
* His DD Form 214
* His social security account number (SSAN) card
* A death certificate

CONSIDERATION OF EVIDENCE:

1.  His records show he enlisted in the Regular Army on 14 April 1983.  His 
DD Form 4 (Enlistment/Reenlistment Document – Armed Forces of the United States) shows his SSN as “xxx-xx-xx3x.”

2.  His record also includes a DD Form 93 (Record of Emergency Data) and permanent orders which show his SSN as “xxx-xx-xx3x.”



3.  The applicant provided a copy of the FSM’s course completion certificate for basic combat training.  This document lists his SSN as “xxx-xx-xx3x.”

4.  He was honorably released from active duty on 13 April 1987 and transferred to the U.S. Army Reserve Control Group (Reinforcement) for completion of his Reserve service obligation.  Item 3 of the DD Form 214 shows his SSN as “xxx-xx-xx2x.”

5.  She provided a copy of the FSM’s SSAN card and death certificate.  Both documents show his SSN as “xxx-xx-xx3x.”

6.  Army Regulation 635-5 (Separation Documents) establishes the standardized policy for preparing and distributing the DD Form 214.  The regulation states the purpose of the separation document is to provide the individual documentary evidence of his or her military service.  It is important that information entered on the form be complete and accurate.  

DISCUSSION AND CONCLUSIONS:

1.  The military personnel file shows the FSM’s SSN is “xxx-xx-xx3x,” and this is the SSN that was consistently used throughout his military career.  

2.  Therefore, his DD Form 214 should be corrected to show the SSN as it is listed on his DD Form 93.

Department of the Army records of the individual concerned be corrected by deleting the entry in item 3 of his DD Form 214 and listing his SSN as shown on his DD Form 93.
