APPLICANT CONTENDS THAT:
He should not have taken the test until 19 Mar which is 42 days
after the date arrived on station (DAS) per AFI 36-2905, page
27, para 4.4.4.
In support of his request, he provides a copy of AFI 36-2905,

Suspect/Witness/Complainant, dated 16 Mar 12; and his AFFMS
printout, dated 26 Jun 12.
The AF IMT 1168, Statement of Suspect/Witness/Complainant, dated
16 Mar 12, prepared by the individual who scheduled him for the
FA, strongly urges the Board to expunge his 15 Mar 12 FA from
the AAFMS because he incorrectly scheduled him prior to the 42
days required by AFI 36-2905.

STATEMENT OF FACTS:
The applicant is currently serving in the Regular Air Force in
the grade of senior airman (E-4).
On 6 Feb 12, the applicant arrived on station at McChord AFB,
WA.
On 15 Mar 12, he failed the contested FA, attaining a score of
63.60, which constitutes an “unsatisfactory” rating.

On 23 Apr 12, AFPC/DPSIM sent the applicant a memorandum
requesting support documentation within 30 days. Specifically,
he was asked for a copy of his Permanent Change of Station (PCS)
orders and Initial Duty Assignment (IDA) worksheet.
The remaining relevant facts pertaining to this application are
contained in the letter prepared by the appropriate office of
the Air Force, which is attached at Exhibit C.

AIR FORCE EVALUATION:
On 26 Jun 12, AFPC/DPSIM recommended disapproval, indicating the
applicant didn’t provide the additional documentation requested
on 23 Apr 12.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
On 8 Aug 12, the applicant provided the documentation requested
by AFPC/DPSIM, i.e., his IDA Worksheet, dated 31 Jul 12; and his
PCS orders #2026002, dated 26 Jan 12, which verify his DAS was
6 Feb 12 (Exhibit D).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by
existing law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice. After a
thorough review of the evidence of record and the applicant’s
complete submission, we believe a preponderance of the evidence
supports corrective action. We note AFPC/DPSIM recommends
disapproval based on missing documentation, however the
documentation was later provided verifying his DAS was 6 Feb 12.
We find the evidence presented is sufficient to support the

removal of the FA dated 15 Mar 12. Therefore, we recommend his
records be corrected as indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air
Force relating to the APPLICANT be corrected to show that his FA
dated 15 Mar 12 be declared void and removed from the AFFMS.
