APPLICANT REQUESTS THAT:
His Fitness Assessment (FA) dated 29 June 2012 be removed from
the Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT:
The incorrect test score was entered on his assessment. He
realizes the importance of overlooking documents before signing
them. Unfortunately, he is unable to locate the person who
annotated his score sheet. He has retaken the test and has
passed it with a score of 89.9. It is important to his career
that the failure is removed from his record.

STATEMENT OF FACTS:
The applicant is currently serving in the Regular Air Force in
the grade of airman first class.
The applicants last five FA scores are as follows:

AIR FORCE EVALUATION:
AFPC/DPSIM recommends denial. DPSIM states upon completion of
his 29 June 2012 FA, the applicant signed and dated the score
sheet. By his own admission, he did not review the score sheet
for correctness. As a result, the FAC entered the score into
AFFMS as unsatisfactory.
IAW AFI 36-2905, paragraph 1.20.2.5, FAC documents FA results on
a hard copy score sheet, signs the score sheet, and obtains
members signature on the score sheet, acknowledging run/walk
time, abdominal circumference measurements, and muscular fitness
repetitions. The applicant failed to provide evidence that
indicates the data entered into AFFMS was not valid. On
18 January 2013, additional documents were requested from the
applicant; however, documents were not received by this office.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
On 25 January 2013, a copy of the Air Force evaluation was
forwarded to the applicant for review and response within 30 days
(Exhibit C). As of this date, no response has been received by
this office.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to
demonstrate the existence of error or injustice. After
thoroughly reviewing the evidence of record and noting the
applicant's contentions, we are not persuaded the contested FA is
in error or unjust. In this respect, we note the applicant has
failed to provide any documentary evidence in support of his
appeal. As such, we find that he has failed to meet his burden
of establishing the existence of an error or an injustice in the
record. In view of the above and in the absence of evidence to
the contrary, we find no basis to recommend granting the relief
sought in this application.

THE BOARD DETERMINES THAT:
The applicant be notified the evidence presented did not
demonstrate the existence of an error or injustice; the
application was denied without a personal appearance; and the
application will only be reconsidered upon the submission of
newly discovered relevant evidence not considered with this
application.

The applicant did not provide evidence that indicates the run-time was documented incorrectly, nor contested at time of FA. The statement provided from his Unit Fitness Program Manager is noted; however, in the absence of a signed fitness assessment score sheet and signed fitness questionnaire, we find insufficient evidence to warrant disturbing the record. On 5 May 2014, the Fitness Assessment Appeals Board (FAAB) considered and denied the applicants request to remove the FA dated 14 February 2012 from the AFFMS.
