APPLICANT REQUESTS THAT: 

 

The Fitness Assessment (FA) dated 5 Jan 12 be removed from the 
Air Force Fitness Management System (AFFMS).

APPLICANT CONTENDS THAT: 

 

His FA failure was determined by a medical provider as being the 
result of a current medical condition he had at the time of the 
assessment. 

 

In support of his appeal, the applicant provides copies of an 
AF Form 108, Physical Fitness Education and Intervention 
Processing, signed by his medical provider on 9 Jun 12; 
AF Form 422, Notification of Air Force Member’s Qualification 
Status, dated 12 Jan 12, and his FA Score Card. 

 

The applicant’s complete submission, with attachments, is at 
Exhibit A.

STATEMENT OF FACTS: 

 

The applicant is currently serving in the Regular Air Force in 
the grade of technical sergeant (E-6). 

 

On 5 Jan 12, he received an unsatisfactory score on his FA.

AIR FORCE EVALUATION: 

 

AFPC/DPSIM recommends approval, stating, in part, that the 
cardio component should be updated to reflect “exempt” in the 
AFFMS. DPSIM notes the applicant was given an AF Form 422 
exempting him from all components of the FA except the abdominal 
circumference.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the 
applicant on 7 Aug 12 for review and comment within 30 days. As 
of this date, no response has been received by this office 
(Exhibit C).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice warranting 
corrective action. We note the Air Force office of primary 
responsibility (OPR) recommends the applicant be exempt from all 
components except the abdominal circumference. Although the 
relief recommended by the OPR will result in a satisfactory 
fitness level, based on our review of the AF Form 422, noting 
the various restrictions, it is our opinion that removing the 
entire FA provides the applicant the more appropriate relief. 
Therefore, in the interest of justice, we recommend the 
applicant’s record be corrected as indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to APPLICANT, be corrected to show that the 
Fitness Assessment, dated 5 Jan 12, be removed from the Air 
Force Fitness Management System.
