APPLICANT REQUESTS THAT: 

 

The following documents be expunged from her records: 

 

1. The Joint Service Achievement Medal (JSAM). (Administratively 
corrected). 

 

2. The American Nurses Credentialing Center (ANCC) certificate 
for the period 1 January 2010 to 31 December 2014. 

 

3. Her Officer Performance Report (OPR) rendered for the period 
4 November 2008 to 3 November 2009, and her ANCC for the period 
29 October 2009 to 28 October 2014, be included in her Officer 
Selection Record (OSR). 

 

4. Her records be considered by Special Selection Board (SSB) by 
the Calendar Year 2009D (CY09D) Major Central Selection Board 
(CSB).

APPLICANT CONTENDS THAT: 

 

She never participated in a joint service exercise or campaign and 
there is no available evidence to substantiate her being awarded 
the JSAM. The dates on her ANCC are incorrect. Her OPR rendered 
for the period 4 November 2008 to 3 November 2009 was not closed 
out until December 2009, after the board met; therefore, it was 
not included in her promotion record. The OPR was not completed 
in time for the board because it kept being returned for 
corrections/revisions. In addition, a copy of her Mental Health 
Nursing Certification was not included as she was pending 
completion of her Master’s Degree in December 2009. 

 

In support of her appeal, the applicant provides copies of 
electronic communications, the erroneous ANCC Certificate, the 
correct ANCC Certificate, and her OPR for the period 4 November 
2008 to 3 November 2009.

STATEMENT OF FACTS: 

 

The applicant is currently serving in the Regular Air Force in the 
grade of captain with a date of rank of 5 June 2003. She has a 
Total Active Federal Military Service Date of 13 October 1997 and 
a Total Active Federal Commissioned Service date of 13 October 
2000. 

 

The applicant has one nonselection to the grade of major by the 
CY09D Major CSB which convened on 1 December 2009.

AIR FORCE EVALUATION: 

 

AFPC/DPAMN recommends approval for SSB consideration; however, 
defers to AFPC/DPSOO to determine if inclusion of the applicant’s 
3 November 2009 OPR in her OSR is appropriate. DPAMN states that 
during their nonselect counseling with the applicant they noted 
that her Officer Selection Brief (OSB) indicated that she was not 
board certified. During their discussion it was determined that 
the applicant had submitted both ANCC Certificates, and their 
office had stamped and submitted the wrong one for inclusion in 
her OSR. In addition, it was discovered the applicant had not 
submitted the appropriate document to add the “M” prefix 
reflecting her certification until March 2010. Therefore, her OSB 
correctly reflected she was not certified because she was not 
carrying the “M” prefix until March 2010. Her record was updated 
on 25 March 2010 to reflect her Primary Air Force Specialty Code 
(PAFSC) as M46P3 with an effective date of 29 October 2009. In 
addition, even though the applicant requests an SSB because “she 
was pending completion of her Master’s Degree in 2009, the OSR 
correctly reflected her education as a Bachelor’s Degree.

AFPC/DPSOO recommends approving SSB consideration with inclusion 
of the 28 October 2014 ANCC Certificate in her OSR and the removal 
of the JSAM from her CY09D OSB. In addition, to make her record 
as whole as possible, they recommend that the “no” in the Board 
Certified Block on the CY09D OSB be changed to “yes.” However, 
they recommend denial to include her 3 November 2009 OPR in her 
CY09D OSR. It is not realistic to hold the Air Force accountable 
for expedition of reports that close out in close proximity to the 
promotion boards. The applicant’s OPR that closed out on 
3 November 2009 was not required to be filed in her OSR until 
3 January 2010. Since many officers fall into this predicament, 
approving SSB consideration with a report not required to be on 
file would generate unfairness in the current promotion process. 

 

DPSOO states that upon review of the applicant’s OSR, it was noted 
the erroneous ANCC Certificate was replaced on 16 April 2010 by 
the ANCC Certificate rendered for the period 29 October 2009 to 
28 October 2014. In addition, the applicant’s PAFSC was updated 


with an effective date back to 29 October 2009, prior to the 
convening of the board.

APPLICANT'S REVIEW OF AIR FORCE EVALUATIONS: 

 

Copies of the Air Force evaluations were forwarded to the applicant 
on 10 September 2010 for review and response within 30 days. As of 
this date, this office has received no response.

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by 
existing law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice. We concur 
with the Air Force offices of primary responsibility that the 
applicant’s record be considered by an SSB for the CY09D Major 
CSB with inclusion of her ANCC certificate for the period 
29 October 2009 to 28 October 2014 in her OSR; and, removal of 
the JSAM from her P0409D OSB. In addition, we concur with 
AFPC/DPSOO that the Board Certified Block of the P0409D OSB be 
changed from “no” to “yes” in order to make the applicant’s 
record as whole as possible. 

 

4. Insufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice in regard to 
including her OPR for the period 4 November 2008 through 
3 November 2009 in her P0409D OSR. We note that this OPR was 
not required to be in her OSR until 3 January 2010, after the 
promotion board convened on 1 December 2009. Based on the 
foregoing, we recommend the applicant’s records be corrected to 
the extent indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air 
Force relating to APPLICANT be corrected to show that: 

 

a. The American Nurses Credentialing Center (ANCC) 
certificate for the period 29 October 2009 to 28 October 2014 
was accepted for file in her Officer Selection Record on 
1 November 2009. 

 


b. The JSAM be removed from her P0409D OSB. 

 

c. The Board Certified Block on her P0409D OSB be changed 
from “no” to “yes.” 

 

d. The corrected record be considered for promotion to the 
grade of major by an SSB for the CY09D Major CSB.
