APPLICANT REQUESTS THAT:
The AF Form 475, Education/Training Report (TR) rendered for the
period 2 October 2008 through 27 February 2009 be replaced with
the attached report.

APPLICANT CONTENDS THAT:
The Distinguished Graduate (DG) criterion for classes 09-01 and
09-02 was not adjusted to reflect the changes in the syllabus.
After notifying the 97th Training Squadron registrar of the
oversight, they corrected his TR to reflect DG. He also was
instructed to complete a DD Form 149, Application for Correction
of Military Records, to officially change his record.
In support of his request, the applicant provides copies of his
corrected AF Form 475, his Distinguished Graduate Certificate,
letters from the 97th Training Squadron (97 TRS) Registrar and
the Commandant.
The applicants complete submission, with attachments, is at
Exhibit A.

STATEMENT OF FACTS:
The applicant is currently serving on active duty in the grade of
first lieutenant.
The applicant attended and was identified as a DG from the KC-135
Pilot Initial Qualification Course from 2 October 2008 through
27 February 2009.
On 29 June 2009, the 97 TRS notified the Evaluation Reports
Appeals Board (ERAB) that the TR has a different evaluator
because the evaluator who signed the erroneous TR is no longer in
that position and not available for signature.

AIR FORCE EVALUATION:
AFPC/DPSIDEP recommends denial. DPSIDEP states the applicant
must provide a new TR signed by the original evaluator or
supporting documentation, directing the changes to his TR which
is already a matter of record. Without the individual who signed
the original TR, it is against AF policy to substitute a report
that does not contain the original evaluator, especially when the
original evaluator may still be contacted.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
A copy of the Air Force evaluation was forwarded to the applicant
on 28 May 2010 for review and comment within 30 days. As of this
date, this office has received no response (Exhibit C).

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing
law or regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to
demonstrate the existence of an error or injustice warranting
favorable consideration of the applicants request. Due to an
administrative error, the applicant's training report ending
27 Feb 09 did not reflect that he was recognized as a
distinguished graduate from the KC-135 Pilot Initial
Qualification Course. The registrar has verified this
information and submitted a new training report reflecting his
distinguish graduate honors. While DPSIDEP states the applicant
must provide a training report signed by the original evaluator,
we note that the previous evaluator is no longer available.
Therefore, in the interest of justice, we believe the training
report should be replaced with the provided reaccomplished
training report. In view of the above, we recommend his records
be corrected to the extent indicated below.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force
relating to APPLICANT, be corrected to show that:
a. The AF Form 475, Education/Training Report (TR),
rendered for the period 2 October 2008 through 27 February 2009,
be, and hereby is, declared void and removed from his records.
b. The attached reaccomplished AF Form 475,
Education/Training Report (TR), rendered for the period 2 October
2008 through 27 February 2009, indicating Distinguished
Graduate, be accepted for file in its proper sequence.

Having received and considered the recommendation of the Air Force Board for
Correction of Military Records and under the authority of Section 1552, Title 10, United States
Code (70A Stat 116), it is directed that:
The pertinent military records of the Department of the Air Force relating to APPLICANT
be corrected to show that:
a. The AF Form 475, Education/Training Report (TR), rendered for the period 2 October
2008 through 27 February 2009, be, and hereby is, declared void and removed from his records.
b. The attached reaccomplished AF Form 475, Education/Training Report (TR), rendered for
the period 2 October 2008 through 27 February 2009, indicating Distinguished Graduate, be
accepted for file in its proper sequence.
