APPLICANT REQUESTS THAT: 

 

His DD Form 214, Armed Forces of the United States Report of 
Transfer or Discharge, be corrected to reflect the following: 

 

 a. The Presidential Unit Citation (PUC). 

 

 b. The Air Force Outstanding Unit Award (AFOUA). 

 

 c. The Air Force Longevity Service Award (AFLSA). 

 

 d. The Small Arms Expert Marksmanship Ribbon (SAEMR).

APPLICANT CONTENDS THAT: 

 

Upon receipt of his DD Form 214, he discovered some of his 
decorations and medals were omitted. While serving in the Air 
Force, he was authorized six decorations and medals, of which he 
only received the National Defense Service Medal and the Air 
Force Good Conduct Medal. He was stationed at the 613th Tactical 
Fighter Squadron in Torrejon Air Base, Spain when they were 
awarded the PUC and the AFOUA. He was eligible for the award of 
the AFLSA after completing 4 years of service, characterized by 
the award of the Air Good Force Conduct Medal. 

 

In support of his request, the applicant provides copies of his 
Small Arms Marksmanship Certificate, DD Form 214, a Training 
Certificate, and newspaper articles.

STATEMENT OF FACTS: 

 

The applicant enlisted in the Regular Air Force on 29 May 1965. 
He was progressively promoted to the grade of staff sergeant (E-
5), having assumed that grade effective and with a date of rank 
of 1 February 1969. He was honorably discharged from the Air 


Force on 31 March 1969. He served 3 years, 10 months and 2 days 
of total active service. 

 

The applicant’s DD Form 214 reflects he was awarded the National 
Defense Service Medal (NDSM), and the Air Force Good Conduct 
Medal. 

 

The Secretary of the Air Force awards the AFOUA to numbered units 
that have distinguished themselves by exceptionally meritorious 
service or outstanding achievement that clearly sets the unit 
above and apart from similar units. The services include: 
performance of exceptionally meritorious service, accomplishment 
of a specific outstanding achievement of national or 
international significance, combat operations against an armed 
enemy of the United States, or military operations involving 
conflict with or exposure to hostile actions by an opposing 
foreign force. 

 

The PUC is awarded in the name of the President of the United 
States, to units of the Armed Forces of the United States and of 
cobelligerent nations, for extraordinary heroism in action 
against an armed enemy on or after 7 December 1941. 

 

The AFLSA is awarded to service members who completed four years 
of honorable active or reserve military service with any branch 
of the United States Armed Forces.

AIR FORCE EVALUATION: 

 

AFPC/DPSIDR recommends denial. DPSIDR states the applicant was 
not assigned or attached to a unit that received the PUC or 
AFOUA. In addition, active and Reserve service cannot be 
combined for a total of four years. In regards to his entitlement 
of the AFLSA, the applicant did not complete four years of active 
or Reserve military service.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the applicant 
on 28 May 2010 for review and comment within 30 days. As of this 
date, this office has received no response (Exhibit D).

THE BOARD CONCLUDES THAT: 


 

1. The applicant has exhausted all remedies provided by existing 
law or regulations. 

 

2. The application was not timely filed; however, it is in the 
interest of justice to excuse the failure to timely file. 

 

3. Insufficient relevant evidence has been presented to 
demonstrate the existence of error or injustice. We took notice 
of the applicant’s complete submission in judging the merits of 
the case; however, we agree with the opinion and recommendation 
of the Air Force office of primary responsibility and adopt its 
rational as the basis for our conclusion that, with the exception 
of the award administratively added to his record, the applicant 
has not been the victim of an error or injustice. Therefore, in 
the absence of evidence to the contrary, we find no basis to 
recommend granting the relief sought in this application.

THE BOARD DETERMINES THAT: 

 

The applicant be notified that the evidence presented did not 
demonstrate the existence of material error or injustice; that 
the application was denied without a personal appearance; and 
that the application will only be reconsidered upon the 
submission of newly discovered relevant evidence not considered 
with this application.
