APPLICANT REQUESTS THAT:

The Air Force Form 475, Education/Training Report and Air  Training  Command
(ATC) Form 126A, Record of Commander’s Review Action be  corrected  to  read
"Elimination due to medical concerns."

APPLICANT CONTENDS THAT:

He respectfully requests the AFBCMR correct an error on his AF Form 475  and
AETC 126A. His AF 475, "Education/Training  Report"  dated  26  Dec  02,  in
section 2, block 2, block 4 incorrectly reflects his reason for  elimination
from training as "Drop on Request." His AETC  126A,  "Record  of  Commanders
Review Action," dated 12 Dec 02, section I, also  incorrectly  reflects  the
reason for his elimination from training as  "Drop  on  Request"  and  as  a
result, Section III automatically requires that he "not  be  considered  for
Undergraduate Navigator Training or  Air  Battle  Management  Training."  He
requests the  Board  consider  the  enclosed  information  and  approve  his
request.

In support of  his  request,  he  submits  a  personal  letter,  letters  of
recommendation, medical statements, IG Request, AETC Form  126A,  Record  of
Commander’s Review Package, Training Reports, Feedback Worksheets, AETCI 36-
2205, Medical Management Requirements, and AETCI 48-102, Medical  Management
of Undergraduate Flying Training Students.

Applicant’s complete submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:

Applicant is currently in the Regular Air Force in the grade of Captain.

Applicant entered JSUPT at Vance AFB, OK with Class 03-12  in    July  2002.
After 39.6 hours in the T-37 primary phase, applicant  was  eliminated  from
training.

      a. Reason for his elimination from training shown on the AF Form  475,
and AETC Forms 126A, and 240-5 Summary of Record  of  Training  is  Drop-on-
Request (DOR).

       b.  Applicant's  training  records  indicate  he  had  two   pre-sole
airsickness episodes. Training records provided indicated  he  was  seen  by
the Flight Surgeon's office after each airsickness episode. Records have  no
mention   of   any   prescribed   medication,   relaxation   or    rotation-
desensitization (Barany chair) training.

      c. Applicant states after  his  sixth  airsickness  episode  with  "no
medical help in sight", facing  the  prospect  he  would  be  held-over  two
classes because he was behind his classmates  on  the  training  timeline-he
felt he had no option but to self-initiate his elimination from (DOR).

AIR FORCE EVALUATION:

AETC/A3F  recommends  approval  and  states  there   is   reasonable   doubt
applicable  policies,  instructions  and  management  procedures  were   not
followed in this case. Applicant's AF Form 475, AETC Forms  126A,  and  AETC
240-5, should be changed to show the elimination  cause  as  "administrative
for lack of adaptability". Change the Form 126A recommendation to  "consider
for reinstatement at a later date" and show the  applicant  as  eligible  to
apply for Undergraduate Navigation training or Air Battle Manager  training.
As long as  the  applicant  still  desires  to  fly  and  remains  otherwise
eligible, he should be provided another SUPT  opportunity.  Because  of  the
intervening time interval, he should reenter training from the beginning  of
the course.

AETC/A3F complete evaluation, with attachments, is at Exhibit C.

AFPC/DPPPEP recommends approval to correct the  12  December  2002  training
report if the  AETC  Form  126A  and  AETC  240-5  forms  are  corrected  as
recommended by AETC/A3F. If the decision is to grant relief,  section  4  of
the  12  December  2002  training  report  should  be  corrected   to   read
"Administrative elimination for lack of adaptability.

AETC/A3F  provided  an  advisory  concerning  the  proper  procedures  Vance
officials should have followed. A3F  specifically  recommend  AF  Form  475,
AETC Form 126A and  AETC  240-5  be  changed  to  show  the  elimination  as
"administrative for lack of adaptability.

The 12 December 2002 training report is accurate due  the  member's  current
documentation showing the member  dropped  on  request.  However,  based  on
AETC/A3F's finding and recommendation, AFPC/DPPPEP agrees the report  should
be corrected if the AFBCMR grants the member relief.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:

Copies of the Air Force evaluations were forwarded to  the  applicant  on  5
May 2006 for review and comment within 30  days.   As  of  this  date,  this
office has received no response.

THE BOARD CONCLUDES THAT:

1.  The applicant has exhausted all remedies provided  by  existing  law  or
regulations.

2.  The application was timely filed.

3.  Sufficient relevant evidence  has  been  presented  to  demonstrate  the
existence of  an  error  or  injustice  that  would  warrant  restoring  the
applicant's  eligibility  for  flying  status.   Based   on   the   evidence
presented, it is apparent prescribed airsickness management procedures  were
not followed in this case. Medication was available to  help  the  applicant
during his pre-solo flights, but not prescribed. The Board notes that  there
is  reasonable  doubt  applicable  policies,  instructions,  and  management
procedures  were  followed  in  this  case.   Therefore,  in  view  of   the
foregoing, and in an effort to offset any possibility of  an  injustice,  we
agree with the recommendations of the offices of primary  responsibility  to
correct the applicant's records as indicated below.

THE BOARD RECOMMENDS THAT:

The pertinent military records of the Department of the Air  Force  relating
to APPLICANT, be corrected to show that:

      a. AETC Form 126A, Record of Commander's  Review  Action,  Section  I,
reflects "ADMINISTRATIVE FOR LACK OF ADAPTABILITY"; "Section  III,  reflects
"BE CONSIDERED FOR REINSTATEMENT IN THIS COURSE AT A  LATER  DATE"  and  "BE
CONSIDERED  FOR  UNDERGRADUATE  NAVIGATOR  TRAINING  OR  BATTLE   MANAGEMENT
TRAINING."


      b.  AF Form 475, Section II, item 4, reflects ADMINISTRATIVE FOR  LACK
OF ADAPTABILITY."

MEMORANDUM FOR THE CHIEF OF STAFF

      Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:

        The pertinent military records of the Department of the Air Force
relating to XXXXXXX, XXXXXXX, be corrected to show that:

               a. AETC Form  126A,  Record  of  Commander's  Review  Action,
Section I, reflects "ADMINISTRATIVE  FOR  LACK  OF  ADAPTABILITY";  "Section
III, reflects "BE CONSIDERED FOR REINSTATEMENT IN THIS  COURSE  AT  A  LATER
