APPLICANT REQUESTS THAT:
His DD Form 214 be corrected to reflect he received three Air Force
Achievement Medals (AFAMs) and the Air Force Longevity Service Award.

APPLICANT CONTENDS THAT:
He completed four years of honorable active duty service in the Air Force
and would like his DD Form 214 corrected.
In support of his request applicant provides a copy of his DD Form 214 and
copies of his AFAM and AFAM (First Oak Leaf Cluster) citations. The
applicant’s submission, with attachments, is at Exhibit A.

STATEMENT OF FACTS:
The applicant enlisted in the Regular Air Force in the grade of airman
basic (E-1) on 2 July 1997. He was progressively promoted to the grade of
senior airman (E-4). On 1 July 2001, he was honorably released from active
duty, having served 4 years on active duty. He was transferred to the Air
Force Reserve for completion of his military service obligation (18
September 2004). His DD Form 214 reflects he was awarded the AFAM, Air
Force Training Ribbon, Air Force Outstanding Unit Award and the Air Force
Good Conduct Medal.
As of 10 September 2004, the Military Personnel Data System (MilPDS)
reflects the applicant was awarded an AFAM for the period ending 1 March
2000.
By letter, dated 12 May 2004, the Air Force Reserve Individual Reserve
Liaison Division advised the applicant that copies of the special orders
awarding him the AFAMs would be needed in order to correct his record. The
applicant did not respond. In this same letter, the applicant was advised
that his record was corrected to reflect the Air Force Longevity Service
Award.

AIR FORCE EVALUATION:
ARPC/DPRFQ recommends the application be denied. DPRFQ states they never
received the requested special orders awarding him the AFAMs. ARPC/DPRFQ
evaluation is at Exhibit C.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
On 13 August 2004, a copy of the Air Force evaluation was sent to the
applicant for review and comment. As of this date, this office has
received no response.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Insufficient relevant evidence has been presented to demonstrate the
existence of error or injustice. As indicated by the Air Force office of
primary responsibility, the applicant’s DD Form 214 has been corrected to
reflect award of the Air Force Longevity Service Award; therefore, the only
request before this Board concerns the Air Force Achievement Medals
(AFAMs). The evidence presented does not establish to our satisfaction
that approval of the requested relief is appropriate. On 12 May 2004, the
applicant was asked to provide copies of the special orders for the AFAM
1OLC and AFAM 2OLC. He did not respond. Therefore, in the absence of such
evidence, we agree with the opinion and recommendation of the Air Force
office of primary responsibility and adopt their rationale as the basis for
our conclusion that the applicant has not provided the necessary
documentation to substantiate his claim. Therefore, we find no basis to
recommend granting him the AFAMs.

THE BOARD DETERMINES THAT:
The applicant be notified that the evidence presented did not demonstrate
the existence of material error or injustice; that the application was
denied without a personal appearance; and that the application will only be
reconsidered upon the submission of newly discovered relevant evidence not
considered with this application.
