His AF Form 475 - Education/Training Report (TR) rendered for the period 7
June 2001 through 15 June 2002 be corrected to reflect his correct Duty Air
Force Specialty Code (DAFSC), AFSC, and the period of the “FROM” date be
changed to 5 January 2002.

APPLICANT CONTENDS THAT:
His AF Form 475 is in error. The DAFSC in Block 4 of Section I should read
K12B3D. The AFSC in Block 1 of Section II should read W12B3D. The Period
of Report in Block 6 is incorrect and does not equate to the length of the
course. Period of Report - From date should be corrected to 5 January
2002.

STATEMENT OF FACTS:
The applicant is currently serving on extended active duty in the grade of
major effective and with a date of rank (DOR) of 1 April 2003.
The AF Form 475 Education/Training Report for the period 7 June 2001
through 15 June 2002 block 4, section I reflected a DAFSC of 13B3E and
block 1, section II reflected an AFSC of W13B3E.

AIR FORCE EVALUATION:
AFPC/DPPPE recommended the Board approve the request to correct the DAFSCs,
but deny the request to change the “FROM” date of the report.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION:
The applicant provided a response which is at Exhibit E.

THE BOARD CONCLUDES THAT:
1. The applicant has exhausted all remedies provided by existing law or
regulations.
2. The application was timely filed.
3. Sufficient relevant evidence has been presented to demonstrate the
existence of an error or injustice. With regard to the applicant’s Duty
Air Force Specialty Code (DAFSC) and Air Force Specialty Code (AFSC) on the
contested Education/Training Report (TR), AF Form 475, we note that the
office of primary responsibility has recommended that these be corrected as
requested and this we so recommend. With respect to the “From” date on the
contested TR, we note that according to the governing AFI the “From” date
of the TR should be the day following the “Thru” date of the last Officer
Performance Report (OPR) on file. In the applicant’s case, the last OPR on
file closed out on 7 June 2001 and the TR’s “From” date was recorded as 7
June 2001. However, it appears that an OPR for the period 8 June 2001
through 4 January 2002 was rendered on the applicant but was not a matter
of record at the time the TR was prepared. We have been advised by HQ
AFPC/DPPPEP that the applicant’s Military Personnel Flight (MPF) has a copy
of the OPR closing 4 January 2002 and it will be accepted for file in his
Officer Selection Record (OSR). In view of this situation, they advise
there would be no objection to changing the “From” date on the contested
TR. Therefore, in an effort to offset any possibility of an injustice, we
recommend the applicant’s records be corrected as indicated below. Lastly,
we note the applicant has been promoted in the primary zone to the grade of
major, therefore, promotion consideration by a Special Selection Board is
not an issue with respect to these changes to his record.

THE BOARD RECOMMENDS THAT:
The pertinent military records of the Department of the Air Force relating
to APPLICANT, be corrected to show that the Education/Training Report, AF
Form 475, rendered for the period 7 June 2001 through 15 June 2002, be
amended as follows:
a. Section I, Block 4, DAFSC, K12B3D, rather than 13B3E.
b. Section I, Block 6, Period of Report, reflect 5 January 2002,
rather than 7 June 2001.
c. Section II, Block 1, AFSC/AERO Rating/Degree Awarded reflect W12B3D
B-52 Weapons Instructor, rather than W13B3E B-52 Weapons Instructor.

Having received and considered the recommendation of the Air Force
Board for Correction of Military Records and under the authority of Section
1552, Title 10, United States Code (70A Stat 116), it is directed that:
The pertinent military records of the Department of the Air Force
relating to , be corrected to show that the Education/Training Report,
AF Form 475, rendered for the period 7 June 2001 through 15 June 2002, be
amended as follows:
a. Section I, Block 4, DAFSC, K12B3D, rather than 13B3E.
b. Section I, Block 6, Period of Report, reflect 5 January 2002,
rather than 7 June 2001.
c. Section II, Block 1, AFSC/AERO Rating/Degree Awarded reflect

2. Corrections be made to the Duty History section of his Officer Selection Brief (OSB). A review of the corresponding Officer Performance Reports clearly reflect the duty title data verified by DPAOM6. Notwithstanding the Air Force's argument that his correct duty title was reflected on his OPR we believe that the determination as to whether or not the error on his OSB had a negative impact on his selection status should be made by selection board members.
_________________________________________________________________ AIR FORCE EVALUATION: The Chief, Reports & Queries Section, AFPC/DPAPS1, reviewed this application and indicated that the reviewer for the OPR closing 31 Dec 94 signed as Commander of the USAF Air Warfare Center so “Center” is the correct duty command level for this duty entry. This OPR clearly shows that the duty title was incorrect on the OPB for the 950701 entry; therefore, DPAPS1 changed the duty title for this entry in...
DPAPS1 stated that applicant’s OPR closing 20 Oct 97 reflects the DAFSC as “62E3G.” This is mirrored under his duty history segment on the PDS and is correct based on the above mentioned OPR. Applicant’s request and documentary evidence are at Exhibit A. However, if the Board considers the case on the evidence provided, DPPPA recommended that the DAFSC on the contested report be changed from “4A41” to “41A3.” They did not recommend adding the “C” prefix to the DAFSC. DPPPA stated that each member of an organization is assigned to a duty position number in accordance with the Unit Personnel Management Roster (UPMR).
